This skill runs an adversarial plan-review loop against Codex on a task's drafted plan before that plan is associated with the task and implemented. It is invoked by the plan / next workflows: they draft a plan, hand its path here, and only proceed once this skill reports approval.
The loop lives entirely inside one skill invocation: no Stop hook, no cross-session state. Codex reviews, you address, Codex re-reviews via the same session, until Codex outputs a bare-line APPROVED token or the 20-round cap is hit. The output is a clear approval signal the calling workflow acts on — there is no sentinel file and no gate hook.
Gotchas
- Codex MCP unavailable is fail-warn, not fail-stop. If Codex cannot be reached, report approval anyway with a prominent warning that the plan was NOT reviewed by Codex, and let the caller proceed. A Codex outage never blocks forward progress.
APPROVED must be on a bare line by itself. Codex sometimes quotes the word in the middle of a sentence. Only a trailing line matching ^APPROVED$ counts as approval.
- The skill is repo-scoped. The plan lives under the repo at
tmp/plans/<task-id>.md (or the task's stored plan path). If the caller isn't in a git repo, abort with a clear message.
- Reuse the Codex session across rounds. Round 1 calls
mcp__codex__codex (a fresh session) and captures the conversation/session id it returns. Rounds 2+ call mcp__codex__codex-reply with that session id so the same Codex thread continues.
- Rebuttals are valid round responses. You are not required to accept every Codex finding. A reasoned pushback is a valid response. Do not over-edit the plan to appease findings you disagree with.
- Codex must stay in review-only mode. Every Codex prompt must explicitly forbid skills, tools, agents, hooks, state edits, file edits, branch creation, PR creation, and workflow execution.
Inputs
The plan to review. Source priority:
- Plan file path: the task's plan file, typically
tmp/plans/<task-id>.md or the path stored on the task (tasks get <task-id> → plan). Best — read it verbatim.
- Mid-conversation draft: write it to
tmp/plans/<task-id>.md first, then review that file.

Read the plan file's full text. You'll substitute it into the Codex prompts as <PLAN_TEXT>.
Step 2: Round 1 — start the Codex session
Call mcp__codex__codex with a fresh session, the repo root as the working directory, and xhigh reasoning. Pass the adversarial critic prompt below as the prompt (with the plan text substituted). Capture the conversation/session id the tool returns — rounds 2+ reuse it. Persist Codex's response to tmp/plan-review/round-1.md.

When the plan is genuinely ready to execute, and only then, output exactly:
APPROVED
on a line by itself at the end of your response. Do not output "APPROVED" on its own line unless you have zero blocking concerns.

Two valid responses to each finding:
- Accept and edit the plan: update the plan file at
tmp/plans/<task-id>.md (or the task's plan path) with the Edit/Write tools.
- Rebut: write a reasoned pushback. Rebuttals are valid — Codex can engage with them rather than treating every finding as non-negotiable.
3b. Continue the Codex session
Call mcp__codex__codex-reply with the captured session id and high reasoning. Pass the delta-only round template below as the prompt. Persist the response to tmp/plan-review/round-${N}.md.
