ac-start

1. Read ../aftercare/SKILL.md first and follow its runtime resolution and queue rules.
2. If the user provides a ticket id, run start <id>.
3. If the user asks for the next item or gives no id, call pick-next first and then start <id> when safe.
4. Never bypass the single-active guard; explain if another ticket is already in progress.
5. Return the claimed ticket id, status, and next queued candidate.
