Task Tracker

Ikaros 用这个 direct tool 显式维护未完成任务状态。它不是场景专属 PR tracker，而是通用闭环工具：

• 查看当前用户仍未闭环的任务
• 读取某个任务的 follow-up 上下文
• 把任务标记成 waiting_external
• 显式关闭任务（设为 completed 或 failed 状态）
• 记录 done_when、next_review_after 和外部引用
• 在 heartbeat 自动继续处理前，先给用户发一条简短通知
