Steps

Read .trellis/spec/agent-methodology/debugging.md.
Reproduce the issue.
Read full error output.
Check recent changes.
Gather evidence at component boundaries.
Compare against similar working code.
State one hypothesis.
Test one variable.
Create a failing regression test when possible.
Fix root cause only.
Send result to verification gate.
