论文写作过程中，将产生的答辩预判问题自动格式化并追加到 Obsidian 答辩预设文件。
/defense <问题描述>/defense （无参数时，从当前对话上下文自动提取）用户直接给出问题和回答，skill 负责格式化并追加。
用户在讨论中产生了可作为答辩问题的分析（如"为什么 X 而不是 Y"），skill 从对话上下文中提取：
1. 确定目标文件路径
2. 读取现有文件，获取当前最大 Q 编号
3. 格式化新 Q/A 条目
4. 追加写入文件
5. 输出确认信息
