Correctness

• Is the logic correct in all scenarios?

Security

• Injection risks — SQL, command, XSS?
• Secrets — are credentials exposed?
• Permissions — is access properly controlled?

Maintainability

• Is the code readable?
• Are names meaningful?
• Is complexity justified?
• Are there comments where needed?

Performance

• Any obvious inefficiencies?
• N+1 queries or loops?
• Memory leaks potential?
• Blocking operations in async code?

Testing

• Is the code testable?
• Are tests included?
• Do tests cover edge cases?

Severity levels:
