Plan (Scribe) → Build (Builder) → Review (Overseer)
                      ↑                    ↓

• Planning produces a Phase folder on disk
• Building modifies code and updates progress
• Review either declares the Phase complete OR adds follow-up steps and sends back to Build

No Phase is complete without Overseer approval.
