Steps

1. Update the one source of truth.
2. Link from docs/00_INDEX.md if the doc is permanent.
3. Avoid duplicating text from other docs.
4. Update examples when behavior changes.
5. Remove stale TODOs and dead docs.
6. Run docs checks when available.

Good docs

Good docs help the next agent find the right context quickly. They do not try to include everything.
