Tonto

Release-and-publish playbook for the Tonto monorepo. Bumps a patch version on the three publishable packages (tonto-cli, tpm, extension), publishes them to their registries (npm for tonto-cli + tpm; VS Code Marketplace + Open VSX for the extension), and cuts a GitHub release with the build artifacts attached.
