- State current behavior, decisions, constraints, and unresolved issues directly.
- Turn lasting lessons into concise current rules without retaining the obsolete error story.
- Remove obsolete material instead of layering new warnings on top of it or archiving it by default.
- Move history only when it has ongoing value for traceability, retrospectives, migrations, or decisions, and keep it outside the default active context.
