Flag semantics (2026-04-17):

Why default is LOOP: agents and humans consistently forgot to run re-review after a single-pass fix, leaving suggestion-severity issues unresolved in merged PRs. Making LOOP the default satisfies the ~/.claude/CLAUDE.md Post-Implement Hard Rule ("0 issues all severities before merge") without requiring the caller to remember the orchestration steps.

Mission

Apply fixes for all issues found by prp-review or prp-review-agents:

1. Load the review artifact and parse issues by severity
2. Detect project toolchain and validation commands
3. Checkout the PR branch (or use it if already on it)
4. Fix issues in priority order: Critical → High → Medium → Suggestion
5. Run validation after each severity batch
6. Commit and push fixes to the PR branch
7. Post a summary comment on the PR
8. (Loop mode only) Re-review → if issues remain, loop back to step 1 with the new artifact; if 0 issues, exit; if MAX_ROUNDS exceeded OR ALL_SKIPPED for 2 rounds, escalate.

Golden Rule: Fix what the review found, plus same-pattern siblings across all files changed in this PR. Don't refactor unrelated code. If a fix is unclear or risky, skip and note it.

Format-specific notes:

• agents-review: Does not use "Medium" — issues are Critical, Important, or Suggestion. Treat Important as High.
• Codex adapters: Use parenthetical labels (e.g., "Critical (block merge)"). Match on the keyword before the parenthetical.
• Standard review: Uses "Pass" column (pass name). Agents-review uses "Agent" column (agent name). Both are valid source identifiers.

Severity promotion for test gaps:

• Test gaps flagged as "NOT TESTED" that cover security guarantees (auth, PKCE, token handling, encryption, access control, input validation) → promote to Critical regardless of original label.
• Other test gaps → keep original severity but do not skip — write the missing tests as part of the fix.

3.2 Group by File (Within Each Batch)

For efficiency, group issues by file within each severity batch so each file is read once per batch. If the same file appears in multiple severity batches (e.g., Critical and High), it will be read again in the later batch — this is acceptable because the file content may have changed from earlier fixes.

Phase 4: FIX — Apply Fixes

Process severity batches in order: Critical → High → Medium → Suggestion

Before processing any issues, initialize an empty Pattern Expansions log (list of rows for the Phase 8 table). Append to it during step 3 of each fix. After all batches complete, this log populates the Phase 8 Pattern Expansions table.

4.1 Fix Loop (per severity batch)

For each issue in the batch:

1. Read the file — understand current context around the flagged line
2. Apply the fix — exactly what the review recommends, using the review's suggestion and the file's existing patterns
3. Pattern Expansion — if the fix addresses a recurring pattern-class bug (same type of bug at 2+ locations, e.g., missing type guard, falsy-value blind spot), grep ALL files changed in this PR for other instances of the same pattern and fix them in the same batch. This is NOT refactoring — it is completing the same fix class. If more than 10 sibling instances are found, add ALL to the skip log with reason: "Large pattern expansion (N instances) — requires manual review" and continue without applying sibling fixes. Always log the result:
   • Siblings found and fixed: add row to Phase 8 Pattern Expansions table — Action = "Fixed N"
   • No siblings found: add row to Phase 8 Pattern Expansions table — Action = "No siblings found"

4. Read additional files only if needed — only if the fix requires understanding an import, caller, or type defined elsewhere. Do NOT speculatively read unrelated files.
5. Note deviations — if you must deviate, document why

Rules:

• Fix what the review flagged + same-pattern siblings across all PR files
• Test gaps are code fixes — when the review flags missing tests (e.g., "NOT TESTED", "no test for X", "test gap"), write the missing tests as part of the fix. Test gaps covering security guarantees (auth, encryption, access control, input validation) should be treated as Critical regardless of how the review labeled them.
• Don't refactor surrounding code or fix unrelated issues
• Match existing code style
• If a fix is ambiguous or risky → SKIP and add to skip log

4.2 Skip Logic

Skip an issue if:

• The fix recommendation is unclear
• The fix requires architectural changes beyond the issue scope
• The code has changed since the review (drift detected)
• The fix would break other things

When skipping: Note the issue in the skip log with reason.

4.3 Quick Check After Each Batch

After fixing all issues in a severity batch, run validation based on batch severity:

For Critical and High batches — run type-check + lint + tests:

Running tests after critical/high batches catches regressions early — a test failure after all 4 batches is much harder to bisect.

If test run is slow (>2 minutes for critical/high quick-check): fall back to type-check + lint only for that batch. Note in the fix summary: "Tests skipped in quick-check (slow project) — deferred to Phase 5 full suite."

If quick check fails after a batch:

1. Identify which fix caused the failure
2. If single fix in batch → revert it, add to skip log with reason: "Validation failed"
3. If multiple fixes in batch and cause is unclear → bisect: revert fixes one at a time (last-to-first) using git checkout HEAD -- {file} for each fix's file(s), and re-run the failing check after each revert until it passes. The last reverted fix is the cause — add it to skip log, re-apply the others.
4. If bisection completes with ALL fixes reverted (i.e., the fixes interact and only fail together) → skip the entire batch, add all fixes to skip log with reason: "Multi-fix interaction — fixes pass individually but fail together. Requires manual resolution."
5. Re-run quick check to confirm clean state
6. Continue to next batch

Run the full suite once after all batches are complete. Tests may have already passed during critical/high batch quick-checks — this final run confirms no cross-batch regressions and adds the build check.

Phase 10: LOOP — Re-review and Repeat (LOOP_MODE only)

SKIP this entire phase if LOOP_MODE = false (i.e., --severity or --single-pass was explicit, OR this is a nested review-fix invocation from run-all).

Numbering note: the document already has Phase 7: PUSH, Phase 8: REPORT, and Phase 9: OUTPUT. The loop runs AFTER all of those as the final optional stage, so it is numbered Phase 10 to avoid collision. Earlier drafts used "Phase 7: LOOP" which collided with "Phase 7: PUSH" — fixed 2026-04-17.

10.1 Decide Re-review Mode

Track which issues review-fix just skipped vs actually fixed, and choose the re-review command accordingly (mirrors run-all Step 6.4, post PR #59):

   If all fallbacks miss (no recognized labels in the repo), gh issue create runs with NO --label flag — the issue is still created and the [escalation] title prefix makes it discoverable via search. Do NOT hard-fail the workflow on missing labels.

10.5 Loop Invariants

• Phase 10 only runs after Phase 6 (commit) succeeds. If commit fails, the loop does not start. (Phases 7–9 — PUSH/REPORT/OUTPUT — run whether or not the loop triggers.)
• Each loop iteration runs Phases 1 → 6 in full for the new artifact. This is the existing single-pass pipeline, re-entered with a fresh artifact.
• ROUND counts distinct (review, fix) pairs. Round 1 is the initial review-fix invocation; round 2 is the first re-review + fix; etc.
• MAX_ROUNDS defaults to 5 (via --max-rounds). The escalation-after-2-all-skipped guard is narrower and catches the "review-fix cannot make progress" case earlier.

Why this design: matches run-all Step 6 behavior exactly (PR gobikom/prp-framework#59), so review-fix used standalone produces the same zero-issues guarantee as review-fix inside run-all. No divergence.

Edge Cases

No review artifact exists

Stop and tell user to run /prp-review {NUMBER} first.

PR branch has conflicts

Branch has conflicts with base. Resolve conflicts first:

Issue already fixed

If code at flagged line already matches the recommendation: skip and track as "already fixed" (distinct from "skipped"). Include in the fix summary:

If file content differs significantly from what review expected:

• Warn: "Code has changed since review. Manual verification recommended."

All {N} issues were skipped (unclear fixes or validation failures).

If LOOP_MODE = true: Phase 10.4 Escalation Guard triggers after 2 consecutive all-skipped rounds — creates a tracked GH issue and exits. Do NOT silently return to caller as if done.

If LOOP_MODE = false (single-pass): Display "Manual review required. See skip log above." and exit normally.

Suggestion-only issues

By default, suggestions are included AND the loop continues until they are resolved. To fix only critical/high and skip suggestion-severity entirely:

Note: passing --severity implicitly disables LOOP_MODE (see Input → Flag semantics). To fix Critical/High first with a loop but without Suggestions, run --severity critical,high,medium once, then run review-fix again without flags to enter loop mode for whatever remains.
