CI Failure Triage and Falsification
Objective
Convert an observed CI symptom into a reproducible causal explanation, or a precisely bounded blocked result. Do not patch the log line before understanding the execution graph and baseline.
Evidence intake
Capture before rerun or editing:
- repository, workflow, run ID/attempt, event, actor, ref/SHA, base/head, merge queue context;
- job/step, runner image/version/architecture, environment, service containers;
- exact command, exit code, timestamps, annotations, logs, artifacts, cache hit/miss;
- token/secret availability without exposing values;
- preceding and dependent job states;
- same command at known-good SHA;
- recent workflow/dependency/runner-image changes.
Logs can contain secrets or attacker-controlled terminal sequences. Sanitize display and never paste live credentials.
Classification
Assign one primary class and supporting evidence:
- product/code regression;
- test defect or invalid assertion;
- deterministic configuration/plan/permission failure;
- runner/toolchain/dependency drift;
- external service/network outage;
- resource exhaustion or timeout;
- race/order/flaky behavior;
- cache/artifact corruption or poisoning;
- baseline/pre-existing failure;
- skipped/unreachable/required-check mismatch;
- unknown/blocked.
“Flaky” is not a root cause.
Triage procedure
1. Preserve the original failure
Download allowed logs/artifacts, record hashes where material, and note retention. Do not rerun until the original event/ref/attempt is captured.
2. Reconstruct the job context
Resolve expressions, inputs, matrix values, permissions, environment, checked-out SHA, cache key, artifact producer, and needs state. Confirm the command shown in logs matches the workflow at the executed SHA.
3. Compare against baseline
Use:
- same command at base/previous successful SHA;
- same SHA on another runner or fresh cache;
- focused test outside orchestration;
- dependency/runner image diff;
- first-parent history and the introducing change.
Separate repository defect from platform outage and setup drift.
4. Reproduce with the smallest oracle
Prefer deterministic reproduction:
- unit/contract test for pure logic;
- hermetic Git-DAG or event payload fixture for workflow logic;
- clean environment with cache disabled;
- pinned dependency/tool image;
- controlled concurrency barrier rather than sleeps;
- representative but non-production migration/data fixture.
Record commands and exit codes. If local parity is impossible, state exactly why.
5. Falsify candidate causes
For each candidate identify expected evidence if true and evidence that would disprove it. Change one variable at a time. A rerun that passes proves nondeterminism, not cause.
6. Correct the root cause
- Add characterization/regression protection first where feasible.
- Use the smallest coherent change.
- Do not increase timeout, add retries, clear caches, weaken assertions, or set
continue-on-error unless the failure model justifies it.
- Preserve required-check names and caller interfaces or provide migration.
- Update generated docs/catalog when contract or plan behavior changes.
7. Verify broadly by blast radius
Run focused reproduction, affected suite, lint/type/static/generated checks, integration/contract tests, build/package, then actual CI. Inspect final diff and status after the last edit.
Flake protocol
A flake report must include feasible interleaving/state, frequency sample, seed/order/resource evidence, and a deterministic control if possible. Quarantine is time-bounded, owner-assigned, linked to an issue, and must not turn a required invariant into advisory status.

Prohibited conclusions
- “rerun passed, fixed”;
- “network issue” without dependency evidence;
- “increase timeout” without workload/resource analysis;
- “all CI passes” without the exact observed run/ref;
- hiding a required failure behind an aggregate success job.
