### Step 1: Database Schema
Create users table with:
- id (uuid, primary key)
- email (unique, indexed)
- password_hash (bcrypt with salt rounds = 10)
- created_at, updated_at
- email_verified (boolean, default false)

Rollback: Drop table and indexes
