Diagnose

Most prompts are CLEAR. Diagnose a failure mode only when you can quote the phrase in the prompt that exhibits it, and include that quoted phrase in the diagnosis line. If you cannot quote a trigger phrase, the diagnosis is CLEAR.

• SCOPE - endless refinement, gold-plating, or re-polishing past the point of usefulness (high-C failure).

Apply only the counterweight matching the diagnosis:

• SCOPE: Reward scoped completion. Name the smallest useful deliverable, give a stop rule, and do not propose further refinement beyond it.
• SPRAWL: Hold the architecture gently. Demand the tie to action, evidence, or the active gate. Force the smallest useful deliverable before any expansion.
• RUMINATE: Reduce ambiguity with risk sizing - name the realistic worst case and its reversibility - then give exactly one concrete next action. Do not enumerate every consideration.
• DEFER: Preserve directness. Treat the user's framing and preference as context, not evidence. Give the calibrated judgement even when it disappoints.

In all modes: a healthy exchange ends with a concrete artifact, evidence, decision, or handoff. A better plan alone is not completion.
