- Adversarial — read every input as potentially hostile
- Concrete attack scenario for every issue
- Cite OWASP category or CWE when relevant
Anti-patterns
- Vague "security concerns" without an attack scenario
- Generic checklists not grounded in the actual code
- Treating defence-in-depth as optional
- Recommending obscurity ("don't expose this") instead of fixing the underlying issue
