description	根据 trial_report、review feedback 或用户回修意见，对 chapter_plan_source / scene_plan_source 做 diff-based proposal 回修，调用 plan-reviser 生成 field_patches，并重新生成 scoped planning proposal。Use when plan_revise is needed, trial_report failed, review requests planning changes, or a chapter/scene plan needs a minimal revision.

规划回修

参数

• $0：project_id
• $1：可选，chapter_id，默认 ch001
• $2：可选，target_plan_type，默认 auto；可选 auto / chapter / scene
• $3：可选，scope，默认 auto；chapter 侧可选 alignment / bundle / outline-patch，scene 侧可选 bundle / beat-patch

执行步骤

1. 先读取 projects/$0/workflow/current_stage.json、workflow/progress.json、projects/$0/chapters/${1:-ch001}/planning_manifest.json、chapter_plan_source.json、scene_plan_source.json，以及当前待确认 proposal；如存在 trial_report.json、review_report.json、rewrite_plan.json，一并读取。
2. 结合用户本轮反馈、trial_report.json 暴露的问题、review_report.json 的阻塞项，判断本轮是 chapter 侧还是 scene 侧回修；若 $2=auto，优先让 plan-reviser 决定 target_plan_type。
3. 调用 plan-reviser，输出 target_plan_type、field_patches、change_reason、proposal_scope 或 revision_mode、needs_reconfirm。
4. 只把 field_patches 写回对应的 chapter_plan_source.json 或 scene_plan_source.json，并保留 source_discussion_ref、revision_notes、planning_trace；不要直接 patch 正式 chapter_goal.json、chapter_outline.json、scene_beats.json、context_permission_source.json 或 context_query.json。
5. 如果目标是 chapter 侧，运行 python app.py chapter-room "$0" --chapter-id "${1:-ch001}" --plan-source "projects/$0/chapters/${1:-ch001}/chapter_plan_source.json" --proposal-scope "${3:-bundle}" --discussion-ref "plan-revise:${1:-ch001}"，重新生成 scoped chapter proposal。
6. 如果目标是 scene 侧，运行 python app.py scene-room "$0" --chapter-id "${1:-ch001}" --plan-source "projects/$0/chapters/${1:-ch001}/scene_plan_source.json" --revision-mode "${3:-bundle}" --discussion-ref "plan-revise:${1:-ch001}"，重新生成 scoped scene proposal。
7. 读取新的 pending confirmation，显式核对 proposal_scope、target_paths、planning_manifest.json、chapter_plan_source_ready / scene_plan_source_ready、chapter_projector_primary_source / scene_projector_primary_source、chapter_template_fallback_used / scene_template_fallback_used。
8. 只有在用户明确确认后，才运行对应的 python app.py confirm-stage "$0" chapter_planning --chapter-id "${1:-ch001}" --scope "..." 或 python app.py confirm-stage "$0" scene_planning --chapter-id "${1:-ch001}" --scope "..."。
9. 如果本轮回修的是 chapter 规划且影响了 chapter_goal.json 或 chapter_outline.json，确认后应提示继续 /scene-room；如果本轮回修的是 scene 规划，确认后应提示继续 /compile-context。

阶段产物契约

• 输入文件：projects/$0/chapters/${1:-ch001}/planning_manifest.json、chapter_plan_source.json、scene_plan_source.json、当前 pending confirmation，以及可选的 trial_report.json、review_report.json、rewrite_plan.json。
• 中间产物：target_plan_type、field_patches、change_reason、proposal_scope / revision_mode、needs_reconfirm。
• 输出文件：不直接写正式 chapter/scene contract；而是通过 chapter-room 或 scene-room 重新生成新的 pending proposal，待用户确认后再正式落盘。
• 禁止行为：不要直接 patch 正式 chapter/scene contract；不要把自由文本反馈直接当 canonical planning source；不要跳过 chapter-room / scene-room projector；不要跳过 confirm gate；不要回退到 source_assets/story_setup/story_core_asset.json；不要把 trial_report.json 或 review_report.json 里的问题直接复制成正式合同字段。
