2. Context the Agent Needs
Before conducting the PIR, gather or confirm:

3. Process
Step 1: Blameless Retrospective
The PIR must follow a blameless methodology. The objective is to understand what happened and improve systems and processes, not to assign fault to individuals.
Blameless retrospective principles (derived from Etsy/Netflix safety culture models, reinforced by NIST SP 800-61 guidance):
Assume good intent. Every person involved made decisions based on the information available to them at the time. Hindsight bias distorts our assessment of past decisions.
Focus on systems, not people. Ask "what allowed this to happen?" not "who caused this?" If a human error contributed, ask what system condition made that error possible, likely, or undetectable.
Encourage honest reporting. If responders fear blame, they will withhold information about mistakes, delays, or wrong decisions made during the response. This hides exactly the information most valuable for improvement.

When the PIR focuses on who made mistakes rather than what systemic conditions enabled the incident, participants become defensive, withhold information, and the organization learns nothing.
