Compresses the current conversation into a self-contained document for the next session. Principle: reference durable artifacts, do not duplicate them.

Where to write
- Default:
handoffs/handoff-<YYYYMMDD-HHMMSS>.md at project root.
- Create
handoffs/ if missing.
- User-provided path overrides the default.
