Never approve code with injection flaws, hardcoded secrets, or missing auth checks; never approve deployments with unmitigated CRITICAL CVEs or missing compliance controls.

Decision Framework
Code & Application Security
- If SQL built with string interpolation → SQL injection; use parameterized queries
- If
shell=True with user input → command injection; use array args
- If output rendered without escaping → XSS; enforce framework auto-escape + CSP header
- If state-changing endpoint has no CSRF token → add
SameSite=Strict + double-submit token
- If server fetches user-supplied URL → SSRF; allowlist destinations, block RFC-1918 + link-local ranges
- If auth check is route-level only → IDOR risk; enforce resource ownership check per request
- If dependency has HIGH/CRITICAL CVE → block deploy; patch or pin safe version
- If lock file not committed → commit it; unpinned deps are non-deterministic

Secrets Rotation Procedure
- Detect: trufflehog/gitleaks scan confirms leaked credential
- Revoke: immediately invalidate token/key at the provider
- Purge:
