You do not write generic rotation policies that say "rotate quarterly". You name the specific rotation cadence per credential type (RDS: 30 days; JWT signing keys: 90 days; third-party API keys: per provider best practice; access keys: zero days because they shouldn't exist). You name the specific service that performs the rotation. You name the specific verification command.

JWT Signing Key Rotation
If JWT signing uses RS256 / ES256 / EdDSA (asymmetric), rotation is straightforward:
- Store signing keys in KMS (AWS), Key Vault HSM (Azure), or Cloud KMS (GCP) — never in app code or env vars.
- Embed a
kid (Key ID) header in every JWT.
- Publish active and previous public keys via a JWKS endpoint (
/.well-known/jwks.json).
- Rotation procedure:
- Generate new key pair in KMS
- Add new public key to JWKS endpoint
- Wait one TTL window (e.g., 1 hour for access tokens) for caches to refresh
- Switch signing to new key
- After 7 days (or longest refresh token TTL), remove old public key from JWKS
Rotation cadence: 90 days for production signing keys.
If JWT signing uses HS256, the shared secret has the same risk as a database password. HS256 is acceptable only when the issuer and verifier are the same process. For any multi-service architecture: migrate to RS256/ES256.
Phase 4 — Token Lifecycle
Application tokens (not infrastructure credentials) need defined TTLs and refresh strategies. The defaults below are the industry-standard for 2026; deviation requires a documented reason.

Refresh Token Reuse Detection
When refresh tokens are rotated on every use, detect reuse — if an old refresh token is presented, it means either a replay attack or a race condition. Either way, revoke the entire token family:
1. Client presents refresh_token_v1.
2. Server validates, issues access_token + refresh_token_v2,
marks refresh_token_v1 as "used".
3. Client (legit) uses refresh_token_v2 next time.
4. Attacker presents refresh_token_v1 (stolen earlier).
5. Server sees v1 marked "used" → REVOKE all tokens in the family
(v1, v2, all future siblings), force re-auth for the user.
This pattern is built into most OAuth libraries (Auth0, Okta, Auth.js) — verify it's enabled.
Anti-Patterns
- 24-hour access tokens — common in 2010s SaaS, no longer acceptable. Reduce to 15-60 minutes.
- Non-rotating refresh tokens — defeats the purpose; an exfiltrated refresh token is valid until manual revocation.
- JWT with
exp in the year 2099 — seen in real production. Reject anything > 1 hour for access tokens.
- Storing access tokens server-side — they should be ephemeral in client memory; if they leak, they expire in 15 min.
Phase 5 — Revocation Capability
Every credential type must have a documented revocation procedure. The question is not if you will need to revoke a credential — it is when.

Building a JWT Revocation Strategy
Pure stateless JWTs (no revocation) are acceptable only when access token TTLs are very short (≤15 min) and you accept up to 15 min of unauthorized access in the worst case. For most apps, that's fine.
For higher-security needs (immediate revocation on logout, admin-forced session kill, security incident response):
- Short TTL + introspection: Verifier hits an introspection endpoint per request. Stateful, but allows instant revocation. Cost: latency + introspection endpoint scale.
- Token revocation list (TRL): Maintain a list of revoked
jti values, check per request. Cache the TRL aggressively (1 min TTL). Works well at moderate scale.
- Refresh-token-driven: Access tokens are short-lived (5-15 min). Revocation is implemented at the refresh layer — when refresh is denied, the session ends within 15 min. This is the most common pattern.
Phase 6 — Detection & Alerting
Configure alerts on credential events that indicate misuse or policy violation. The signal-to-noise ratio matters — too many alerts and they get ignored.

For higher-fidelity alerting, send CloudTrail logs to a SIEM (Splunk / Datadog / Sumo / Sentinel) with correlation rules for unusual patterns: "access key created and used from a new IP within 60 seconds" is a higher-confidence signal than either event alone.
Phase 7 — Verification
Concrete commands to verify the credential lifecycle program is working as designed. Run quarterly at minimum; ideally automate as Config rules / Policy assignments.

Investment: ~2 weeks of engineering time. Result: SOC 2 control CC6.1 (logical access controls) now passes with documented evidence. Mean time to revoke a compromised credential dropped from "next business day" to "under 15 minutes" (access token TTL).
Tips for Best Results

Inventory before you remediate. It is tempting to start deleting long-lived keys immediately, but you will break things. Phase 1 (Inventory) is non-negotiable. You cannot remediate what you have not catalogued, and you will be surprised by what is in use.

The dual-secret pattern is the answer for app DB user rotation. Almost every engineer's first attempt at rotation breaks production. Read the AWS Secrets Manager rotation templates, use the alternating-user variant for any non-trivial app, and test rotation in staging before flipping it on in prod.

OIDC federation for CI/CD is a free win. GitHub Actions, GitLab, CircleCI, and Bitbucket Pipelines all support OIDC federation to AWS, Azure, and GCP. There is no good reason to have long-lived CI/CD access keys in 2026. If your CI/CD still uses static keys, fixing this is the highest-ROI work in this skill.

HS256 is fine for single-process services; not for anything else. If your JWTs are signed and verified by the same monolith, HS256 is acceptable. The moment a second service needs to verify, migrate to RS256 / ES256 with JWKS publication. The migration is one-way: never go back.

Rotation requires verification. Configure rotation. Then connect to the DB with the new credential. Then check the app pool refreshed. A rotation that succeeds in Secrets Manager but breaks the app is worse than no rotation — it creates the illusion of safety. Add synthetic monitoring on the auth path.

The 90-day stale credential cleanup is the most important automation. More incidents come from forgotten credentials (former employees, abandoned scripts, dev keys promoted to prod) than from active credential compromise. Build the automation to detect and remove stale credentials, even if every other piece of this skill is deferred.

Provider-specific rotation runbooks are documentation, not code. Stripe, SendGrid, Twilio, and similar services do not all support API-driven rotation. For those, write a clear runbook (manual steps, expected duration, rollback procedure) and store it in the repo next to the IaC. Engineers will reach for it during the actual rotation; do not assume tribal knowledge.

Re-audit after every personnel change. When someone leaves, their human credentials are usually revoked — but the long-lived API keys, service account keys, and shared credentials they generated often live on. The 90-day cleanup catches some; an immediate audit catches more.

Compliance frameworks have specific rotation requirements. PCI-DSS 8.3.9 mandates 90-day password rotation for all account/system credentials in scope. SOC 2 expects documented rotation procedures aligned to your policy (whatever interval you commit to, you must do). HIPAA is less prescriptive but the security risk analysis must address credential management. Match the rotation cadence to the strictest framework in scope.
