Before recommending any major strategic initiative, roadmap phase, or commercial term. Use to validate that risks are understood and mitigated, not to block execution. Run before presenting to client's leadership team.
When NOT to use
Skip for low-risk recommendations (minor process changes, tactical optimizations). Skip if risk assessment was already completed and approved by leadership.

Step 1: Identify Risk Categories
Map risks across 5 domains (Org / Execution / Market / Financial / Technology):
Organizational Risks
- Executive alignment: CEO, CFO, board buy-in; competing priorities
- Culture & talent: Resistance to change; key person departures
- Incentives: Compensation models don't align with strategy
- Decision velocity: Org slow to make decisions; decision-making unclear
Execution Risks
- Scope creep: Deliverables balloon; timeline extends
- Resource constraints: Team too small; competing priorities
- Vendor / partner dependencies: Third-party delays; capacity issues
- Change management: Team untrained or unwilling to adopt new process
Market Risks
- Demand validation: Uncertain market appetite for opportunity
- Competitive response: Competitor launches faster; market shifts
- Regulatory: Compliance requirements change; legal complications
- Customer concentration: Loss of key customer; contract non-renewal
Financial Risks
- Budget approval: CFO blocks investment; insufficient capital

Step 2: Rate Risk Impact & Probability
For each identified risk, score on 5-point scale:
Impact (Business Consequence)
- 5 = Catastrophic (threatens company viability; deal dies; >$10M loss)
- 4 = Critical (major delay; revenue impact $1–10M)
- 3 = High (moderate delay; revenue impact $100K–1M)
- 2 = Medium (minor delay; revenue impact <$100K)
- 1 = Low (negligible impact; process slowdown only)
Probability (Likelihood of Occurrence)
- 5 = Almost Certain (>80% chance; happens if no mitigation)
- 4 = Likely (50–80%)
- 3 = Medium (30–50%)
- 2 = Unlikely (10–30%)
- 1 = Remote (<10%)
Risk Score = Impact × Probability (1–25 scale)
Risk Tier:
- Red (≥16): Critical; requires escalation and active mitigation
- Yellow (8–15): High; requires mitigation plan and monitoring
- Green (≤7): Low; monitor but not blocking
Step 3: Develop Mitigation Strategies
For each Red or Yellow risk, build 2–3 mitigation tactics:
Mitigation Types:
- Prevent: Eliminate root cause (best case)
- Reduce: Lower probability or impact through proactive action
- Transfer: Shift risk to third party (insurance, vendor contract)
- Accept: Acknowledge risk; plan contingency if it occurs
Step 4: Define Contingency Triggers
For high-impact risks, define the "circuit breaker" — what signal triggers the backup plan:
