After applying and verifying a small fix, create the hotfix record:
node scripts/codex-docs.js hotfix "title" --problem="..." --fix="..." --verification="..."
The script creates .crabshell/hotfix/HNNN-*.md and appends to .crabshell/hotfix/INDEX.md.
