Why This Skill Exists
Tumor markers are serum or tissue biomarkers used for cancer screening, diagnosis, prognosis, treatment monitoring, and recurrence detection. Misinterpretation is extremely common — a single elevated value without trend analysis causes unnecessary interventions, while failure to recognize a significant rising trend delays detection of progression. ASCO, NCCN, and ESMO have published evidence-based guidelines specifying which markers are validated for which clinical uses, and no tumor marker should be used outside its validated context.
Payer audits increasingly scrutinize tumor marker ordering patterns. Ordering non-validated markers (e.g., CA-125 for non-ovarian cancers) generates unnecessary cost and clinical confusion. Inappropriate marker interpretation leads to imaging overuse, biopsy complications, and patient anxiety. This skill ensures markers are interpreted within their validated clinical contexts using established kinetic models and decision thresholds.

Step 1: Confirm Validated Clinical Use for Each Marker

Reject any marker-clinical use combination not supported by ASCO, NCCN, or ESMO evidence-based guidelines. Document the guideline reference for each validated use.
Step 2: Analyze Marker Kinetics and Trends
Single values are rarely interpretable — trend analysis is mandatory.
Marker kinetic principles:
- Doubling time (DT): Calculate using at least 2 rising values: DT = (t × log2) / log(V2/V1). PSA doubling time <6 months after prostatectomy indicates aggressive recurrence.
- Half-life: Post-treatment decline should follow expected half-life: AFP half-life 5–7 days, β-hCG 24–36 hours, PSA 2.2 days post-prostatectomy. Failure to normalize per expected kinetics suggests residual disease.
- Nadir: The lowest post-treatment value. Rising above nadir by a clinically significant margin triggers further evaluation.
- Flare phenomenon: Some markers transiently increase in the first 2–4 weeks of effective treatment (PSA flare on enzalutamide/abiraterone, CA-125 flare on chemotherapy). Do not interpret a flare as progression.
Decision thresholds for action:
- PSA: biochemical recurrence = PSA ≥0.2 ng/mL and rising after prostatectomy; ≥2 ng/mL above nadir after radiation
- CEA: a rise >30% from nadir sustained on repeat testing warrants imaging
- CA-125: GCIG criteria for progression = doubling from nadir confirmed on repeat, or doubling from ULN if nadir not reached
Step 3: Assess Confounding Factors
Every elevated or unexpected marker result must be evaluated for non-malignant causes:

Document each confounder assessed and its status. If a confounder is present, its contribution to the elevation must be considered before attributing the result to malignancy.
Step 4: Generate Interpretive Report
Structure the interpretation as:
- Marker and assay identification: Name, assay platform, laboratory reference range
- Serial values table: All available values with dates, graphed if possible
- Trend analysis: Rising, falling, stable, or fluctuating; kinetic calculations when applicable
- Clinical context: Treatment administered, disease status, staging at last assessment
- Confounder assessment: List assessed confounders with disposition
- Interpretation: Consistent with response / stable disease / possible progression / indeterminate
- Recommended action: Repeat in X weeks, correlate with imaging, no action needed, or escalate to oncologist

- Is each marker being used only within its ASCO/NCCN-validated clinical context?
- Are at least 3 serial data points used for trend analysis (not single-value interpretation)?
- Have confounding non-malignant causes been systematically assessed?
- Is the assay platform consistent across measurements, or has a platform change been flagged?
- Does the interpretation align with concurrent imaging findings?
Quality Audit
Guidelines
- Never interpret a single tumor marker value in isolation — trends define clinical significance
- Always confirm the assay platform has not changed between serial measurements (different assays are not directly comparable)
- PSA values after prostatectomy should be undetectable (<0.1 ng/mL); any detectable PSA warrants further evaluation
- CA 19-9 is unreliable in Lewis antigen-negative patients — document Lewis antigen status when using CA 19-9
- Do not order tumor markers for screening in average-risk populations unless guideline-supported (only PSA has conditional screening support)
- Marker decline kinetics matter as much as absolute values — failure to follow expected half-life after treatment suggests residual disease
- CEA elevation from smoking does not negate the need for evaluation in colorectal cancer patients — the clinical context determines interpretation
- When markers and imaging are discordant, imaging takes precedence for treatment decisions, but discordance should prompt further investigation
