When to Use

Use this skill when the user:

• Wants to understand the structure, columns, and format of a lab report they have received (test name, result, reference range, flag, units) and does not know how to read it
• Asks what a named panel measures in general terms -- "What is a CBC?", "What does a CMP test for?", "What is on a lipid panel?", "What is an A1C?"
• Is preparing for upcoming blood work and wants to know what to expect from the draw process, fasting requirements, or what the ordered tests generally evaluate
• Wants to build a productive list of questions to ask their provider at a follow-up appointment about lab results they have already received
• Asks why certain pre-test conditions matter -- why do I need to fast? why does hydration affect results? why do results differ between labs?
• Asks about the difference between a Basic Metabolic Panel and a Comprehensive Metabolic Panel, or between TSH and a full thyroid panel

Do NOT use this skill when:

• The user asks what treatment, medication, or diet change they should make based on a lab result -- defer entirely to the provider
• The user asks about interpreting genetic testing results (23andMe, carrier screening) -- this is a distinct domain requiring genetic counselor guidance
• The user is describing active symptoms and wants to correlate them with lab values -- this is clinical assessment; refer to a provider immediately
• The user asks about occupational or forensic lab testing (drug screens, alcohol levels for legal purposes) -- different regulatory and interpretation context
• The user asks the AI to fill in or predict what their results "probably are" for a test they have not yet had done

Process

Step 1: Clarify What the User Actually Needs

Before explaining anything, spend one response cycle understanding the user's specific situation. This prevents delivering a general CBC overview to someone who has already Googled that and actually needs help phrasing a question for their cardiologist.

• Ask: Do you have a lab report in front of you, or are you preparing for upcoming lab work?
• Ask: Is there a specific panel name listed at the top of the report (CBC, CMP, BMP, Lipid Panel, thyroid panel, HbA1c)?
• Ask: Are you trying to understand what a test measures generally, or are you trying to figure out how to talk to your provider about specific results?
• If the user shares actual numerical values, do NOT interpret them -- acknowledge the values were received, then pivot to the questions framework (see Edge Cases and Rules)
• Note whether the user mentions fasting status, recent illness, or medications, as these will be relevant when building the questions list

Step 2: Explain the Universal Structure of a Lab Report

Every CLIA-certified laboratory report in the United States follows a recognizable structure regardless of whether it comes from a hospital system, independent lab, or physician office lab. Walk the user through these components:

• Patient header block: Name, date of birth, ordering provider, collection date/time, specimen type (serum, plasma, whole blood, urine). The collection date and time matter because many values (cortisol, insulin, certain hormones) fluctuate across the day.
• Panel grouping: Tests are clustered by the panel ordered (e.g., all CBC components together, all CMP components together). Each cluster typically has a panel header.
• Test Name column: The specific analyte being measured. Some reports use abbreviations (BUN, ALT, Hgb, TSH); others spell them out. Both styles appear on the same report from some labs.
• Result column: The measured value from the user's sample. This is the only number specific to them.
• Reference Range column: The interval established by the laboratory using their specific instruments, reagents, and a reference population. This is almost always a 95th percentile interval -- meaning 5% of completely healthy people will statistically fall outside it on any given day. This is critical for the user to understand.
• Flag column: H (high), L (low), C or CR (critical -- requires immediate provider notification), A (abnormal for non-numeric tests), or blank (within range). Explain that H or L simply means "outside this lab's reference interval," not "something is wrong."
• Units column: Common units include mg/dL (milligrams per deciliter), mmol/L (millimoles per liter), g/dL, U/L (units per liter), mEq/L, cells/uL or K/uL or 10^3/uL (all equivalent ways to express cell counts), %, ng/mL, and mIU/L. Units are not interchangeable -- some countries use SI units (mmol/L) and some use conventional US units (mg/dL), so international reference ranges are not directly comparable.
• Footnotes and lab director signature: The name of the CLIA lab director and accreditation number are required by law on certified lab reports. Some reports include methodology footnotes for specific tests.

Step 3: Explain Each Common Panel in Substantive Detail

Do not just list test names -- explain the physiological domain each test probes and why it is ordered together as a panel. Use "this test measures" and "this relates to" language consistently.

Complete Blood Count (CBC) with or without Differential:

The CBC examines the three major cell lineages produced by bone marrow. It is one of the most commonly ordered panels because it gives a broad view of blood cell production and destruction.

• WBC (White Blood Cell Count): Measures the total number of all immune cells. A differential (CBC with diff) breaks this into five types: neutrophils (the most abundant, first-responders to bacterial threats), lymphocytes (coordinate immune responses and produce antibodies), monocytes (clean up debris, transition to tissue macrophages), eosinophils (involved in allergic responses and parasite defense), and basophils (rare; involved in inflammatory signaling). The differential is reported as both absolute counts (cells/uL) and percentages of total WBC.
• RBC (Red Blood Cell Count): Counts the number of red cells. Typically reported in millions per uL (e.g., 4.5 million/uL written as 4.5 x10^6/uL).
• Hemoglobin (Hgb or Hb): The iron-containing protein inside red cells that binds oxygen. Measured in g/dL. This is often the most clinically watched value on the CBC for anemia assessment.
• Hematocrit (Hct or Crit): The percentage of whole blood volume occupied by red cells. Roughly three times the hemoglobin value in g/dL (a useful cross-check).
• MCV (Mean Corpuscular Volume): The average size of a red blood cell, measured in femtoliters (fL). This is a key descriptor: small cells (low MCV, called microcytic) have different causes than large cells (high MCV, called macrocytic). Used by providers when evaluating the cause of anemia -- but explaining that is the provider's job.
• MCH (Mean Corpuscular Hemoglobin): Average amount of hemoglobin per red cell, in picograms. Generally moves in the same direction as MCV.
• MCHC (Mean Corpuscular Hemoglobin Concentration): Concentration of hemoglobin in a given volume of red cells, in g/dL. A low MCHC in combination with low MCV is a specific pattern providers recognize.
• RDW (Red Cell Distribution Width): Measures variability in red cell size. A high RDW means red cells are not uniform in size -- providers use this in combination with MCV.
• Platelets (PLT): Count of clotting cells, typically 150,000-400,000/uL (reported as 150-400 K/uL). Involved in the first steps of clot formation.
• MPV (Mean Platelet Volume): Average size of platelets. Not always reported; when it is, providers may look at it alongside platelet count.

Basic Metabolic Panel (BMP) -- 8 tests:

The BMP evaluates the chemistry of the blood fluid. It is the "quick chemistry" panel and includes:

• Glucose: Blood sugar at the moment of draw. Fasting glucose (drawn after 8+ hours without caloric intake) and non-fasting glucose have different reference ranges. The report should note fasting status if documented. Typical fasting reference range at most US labs: 70-99 mg/dL. Non-fasting interpretation is more variable.
• BUN (Blood Urea Nitrogen): Nitrogen-containing waste product from protein metabolism, filtered by the kidneys. Typical range: 7-25 mg/dL. Elevated results can reflect many things, which is the provider's job to contextualize.
• Creatinine: Waste product from creatine phosphate breakdown in muscle. Stable production rate makes it a reliable kidney filtration marker. Typical ranges differ by sex and muscle mass. Labs report eGFR (estimated glomerular filtration rate) calculated from creatinine.
• eGFR (Estimated Glomerular Filtration Rate): Calculated from creatinine, age, and sex using the CKD-EPI equation (now the standard since 2021; the MDRD equation was used previously). Reported in mL/min/1.73m². This is a calculated value on the report, not directly measured.
• Sodium (Na): The primary electrolyte governing fluid balance outside cells. Typical range: 136-145 mEq/L.
• Potassium (K): Electrolyte critical for nerve and muscle cell electrical function. Typical range: 3.5-5.0 mEq/L. Hemolysis (red cells breaking during blood draw or handling) artificially elevates potassium -- labs flag this.
• Chloride (Cl): Electrolyte that balances sodium. Typical range: 98-107 mEq/L.
• CO2 (Bicarbonate): Reflects the body's acid-base buffering capacity in the blood. Typical range: 22-29 mEq/L. This is bicarbonate measured in serum, not a gas measurement.

Comprehensive Metabolic Panel (CMP) -- 14 tests:

The CMP includes all 8 BMP tests plus 6 additional tests focused on the liver and protein status:

• ALT (Alanine Aminotransferase): Enzyme concentrated in liver cells. Elevated levels in blood generally indicate liver cell disturbance. Typical range: roughly 7-56 U/L (varies significantly by lab and reference population).
• AST (Aspartate Aminotransferase): Enzyme found in liver, heart muscle, and skeletal muscle. Less liver-specific than ALT. Typical range: roughly 10-40 U/L.
• ALP (Alkaline Phosphatase): Enzyme found in liver, bone, kidneys, and intestinal tissue. Typical range: 44-147 U/L. Reference ranges vary significantly with age and bone growth.
• Total Bilirubin: Breakdown product of hemoglobin from old red cells, processed by the liver. Typical range: 0.1-1.2 mg/dL. Direct (conjugated) and indirect (unconjugated) bilirubin fractions are sometimes broken out separately.
• Albumin: The most abundant protein in blood, produced by the liver. Typical range: 3.4-5.4 g/dL. A marker of both liver synthetic function and nutritional status.
• Total Protein: Sum of albumin plus globulins in the blood. Typical range: 6.3-8.2 g/dL.
• Calcium (Ca): Mineral in the blood, about 40% of which is bound to albumin. Typical range: 8.6-10.2 mg/dL. Some labs also calculate a corrected calcium adjusted for albumin level.

Lipid Panel (Fasting):

Ordered to assess fats and cholesterol circulating in the blood. Standard fasting requirement: 9-12 hours without food (water and plain medications are acceptable). Non-fasting lipid panels are now accepted by some guidelines for total cholesterol and HDL, but triglycerides require fasting for accuracy.

• Total Cholesterol: Sum of all cholesterol carried in all lipoprotein particles. Measured in mg/dL. Not evaluated in isolation -- the distribution among particle types matters.
• LDL Cholesterol (Low-Density Lipoprotein): Most commonly reported as a calculated value using the Friedewald equation: LDL = Total Cholesterol - HDL - (Triglycerides/5). This calculation is inaccurate when triglycerides exceed 400 mg/dL, at which point direct LDL measurement is used.
• HDL Cholesterol (High-Density Lipoprotein): Directly measured. Typical reference ranges: greater than 40 mg/dL (men), greater than 50 mg/dL (women), though these vary by lab and guideline source.
• Triglycerides: Fats circulating in blood. Fasting reference range is typically less than 150 mg/dL at most labs. Non-fasting values are expected to be somewhat higher after meals.
• Non-HDL Cholesterol: Calculated as Total Cholesterol minus HDL. Increasingly used because it captures all atherogenic particles without requiring LDL calculation. Many modern reports include this automatically.
• Total Cholesterol/HDL Ratio: Calculated ratio sometimes included. Providers use multiple metrics from this panel, not any single number -- which reinforces why the provider context matters.

Hemoglobin A1C (HbA1c):

• Measures the percentage of hemoglobin molecules that have glucose attached (glycated hemoglobin). Because red blood cells live approximately 90-120 days, A1C reflects average blood glucose exposure over the preceding 2-3 months.
• Does NOT require fasting -- collected at any time of day.
• Reported as a percentage (%). Some labs also report eAG (estimated average glucose in mg/dL), a calculated conversion.
• Certain conditions affect A1C accuracy: hemolytic anemia (shorter red cell lifespan artificially lowers A1C), sickle cell trait, and iron deficiency anemia (can falsely elevate A1C). Always mention these as potential discussion points with the provider if applicable.
• NGSP-certified laboratory methods are the US standard; IFCC units (mmol/mol) are used internationally and produce different numerical values.

Thyroid Panel:

• TSH (Thyroid-Stimulating Hormone): The primary and most sensitive screening test for thyroid function. Produced by the pituitary gland. Typical reference range: 0.4-4.0 mIU/L (varies by lab). TSH has a log-linear inverse relationship with thyroid hormone -- this is counterintuitive but important: high TSH generally suggests the pituitary is working hard to stimulate an underperforming thyroid, not that there is more thyroid activity.
• Free T4 (Free Thyroxine): The unbound, active form of the primary thyroid hormone. Typical range: 0.8-1.8 ng/dL (varies by assay method significantly -- immunoassay methods differ across labs).
• Free T3 (Free Triiodothyronine): The most biologically active thyroid hormone. Less commonly ordered as a screening test. Typical range: 2.3-4.1 pg/mL.
• Total T4 / Total T3: Measures both bound and unbound hormone. Less commonly used now that free assays are available. Affected by changes in binding proteins (e.g., pregnancy increases thyroid-binding globulin, raising total T4 without reflecting true hormone status).
• Thyroid antibodies (Anti-TPO, Anti-Tg): Sometimes included when autoimmune thyroid disease is being evaluated. Not part of standard screening.

Urinalysis (UA) -- when relevant:

Though not a blood panel, UA appears on many annual physical lab orders:

• Dipstick components: pH, specific gravity, protein, glucose, ketones, blood, leukocyte esterase, nitrites, bilirubin, urobilinogen.
• Microscopic components (if ordered): Red blood cell casts, white blood cell casts, bacteria, squamous cells.
• Results are reported as negative/trace/1+/2+/3+ for dipstick, and as number per high-power field (HPF) for microscopic.
• Contamination (especially in females) is a common source of false positive white blood cells and bacteria. Labs may note "possible contamination."

Step 4: Explain Reference Range Mechanics Thoroughly

This is one of the most important conceptual points for users. Many users assume any value outside the reference range means something is wrong. Correct this misconception carefully:

• Reference ranges are established by measuring the same analyte in a large sample of people considered healthy by the reference lab, then taking the central 95% of those values as the range. This means 1 in 20 healthy people will have a result outside the reference range on any given test simply by statistical probability.
• Reference ranges are specific to the lab's instruments, reagent lots, and calibration. ALT measured on a Roche Cobas analyzer will have a slightly different reference range than ALT measured on an Abbott Architect analyzer.
• Some labs use sex-specific and age-specific reference ranges (hemoglobin, alkaline phosphatase, creatinine, HDL). The report will indicate which reference range applies.
• Ethnicity-based adjustments exist for some calculated values (eGFR was formerly race-adjusted; the 2021 CKD-EPI update removed race as a variable). If the user sees an older report with race-based eGFR, explain this context.
• Reference ranges are NOT therapeutic targets. A provider may have a specific target in mind for a patient that differs from the population reference range -- this is clinical judgment based on the individual's history.
• Critical values (flagged CR or C) are a separate category: values so far outside the reference range that the lab is required to call the ordering provider immediately under CLIA regulations. Common examples include potassium below 2.5 or above 6.5 mEq/L, glucose below 40 or above 500 mg/dL, and platelet counts below 20 K/uL.

Step 5: Build a Customized Questions-for-Your-Provider List

Tailor the questions list to the specific panels the user has. Generic questions are less useful than questions tied to the tests the user actually has in front of them. Structure the list in three tiers:

Tier 1 -- Universal questions for any lab result conversation:

• "Are any results outside what you would expect given my specific health history and medications?"
• "Are there results you would like to monitor over time? Should I come back for repeat testing?"
• "How do these results compare to my previous labs -- are there trends I should be aware of?"
• "Are any values in a range that would change your recommendations for me?"

Tier 2 -- Panel-specific questions:

• For CBC: "If my WBC differential was included, were all five cell types within expected proportions?"
• For CMP/BMP: "Was my glucose drawn fasting, and does that affect how you read it?"
• For lipid panel: "Was I properly fasting before this draw? Does it affect any of these values?"
• For thyroid: "Is my TSH in the part of the range you consider optimal, or just within the broad population range?"
• For A1C: "Is there anything about my red cell health or any condition I have that might affect A1C accuracy?"

Tier 3 -- Follow-up logistics questions:

• "Should I make a follow-up appointment to discuss these, or is a phone message or portal message sufficient?"
• "Are there any results where I should call sooner rather than waiting for my scheduled appointment?"
• "Are there preparation instructions for my next set of labs I should know about?"

Step 6: Explain Pre-Test Factors That Affect Results

Equip the user to have a complete conversation with their provider by explaining what variables can cause results to shift:

• Fasting status: Glucose rises after eating (post-prandial peak typically 1-2 hours after a meal). Triglycerides are significantly elevated for 4-8 hours after eating a fatty meal. Non-fasting LDL calculation is unreliable due to the Friedewald equation's dependence on triglycerides. Most labs request 9-12 hours of fasting for lipid panels and some glucose tests.
• Hydration: Dehydration concentrates the blood, which can artificially elevate hemoglobin, hematocrit, sodium, BUN, creatinine, and albumin. Overhydration has the opposite dilution effect. Drinking adequate water before a blood draw (plain water does not break a fast) is always recommended.
• Exercise: Strenuous exercise within 24-48 hours of a draw can elevate CK (creatine kinase, sometimes included in panels), AST, and LDH. It can temporarily alter WBC counts and affect glucose metabolism. Suggest the user note recent intense exercise when discussing results.
• Medications and supplements: Many medications directly affect lab values. Biotin (vitamin B7), even at doses found in many over-the-counter supplements (5,000-10,000 mcg), interferes with biotin-streptavidin immunoassay technology used in many thyroid hormone and cardiac marker tests, causing falsely abnormal results. This is an FDA-warned interference. The user should always tell their provider about ALL supplements, not just prescription medications.
• Time of day: Cortisol peaks around 8 AM and is lowest around midnight. Iron and TIBC (total iron binding capacity) have diurnal variation. TSH is highest in the early morning. For consistency, many labs collect hormone tests at the same time of day across serial measurements.
• Recent illness or infection: Acute infection causes a reactive increase in WBC and inflammatory markers. CRP (C-reactive protein), if ordered, will be elevated during any inflammatory state. Glucose may be elevated with physiologic stress. BUN and creatinine can rise with fever-related dehydration.
• Tourniquet application time during draw: Prolonged tourniquet use (more than 1-2 minutes) can cause localized hemoconcentration, elevating potassium, total protein, and some enzyme values. Hemolysis from a difficult draw can falsely elevate potassium and LDH. Labs will note hemolyzed specimens on the report.
• Specimen handling: Some tests (like potassium) must be separated from red blood cells promptly; delays in processing can falsely elevate values. Cold agglutinins can falsely lower RBC count. Labs document specimen quality.

Step 7: Deliver the Output and Close With Context-Setting

After providing the panel descriptions, report format explanation, and questions list:

• Remind the user explicitly that the reference ranges and typical values described here are illustrative -- their own report may show slightly different ranges because those are set by their specific lab.
• Remind the user that multiple results are often evaluated together as a pattern, not as isolated numbers -- a single value outside the range means less than a consistent pattern across related tests.
• Offer to help them draft specific questions if they want to share which results were flagged (without asking for the numerical values if they do not want to share them, and without interpreting any values they do share).
• If the user expresses anxiety about flagged results, normalize the statistical reality: given that a standard annual physical CBC + CMP + lipid panel = approximately 25+ individual test results, the probability that at least one will land outside the reference range purely by chance exceeds 70% for a completely healthy person.

### About the Reference Ranges on Your Report

- Reference ranges are set by each individual lab using their own instruments and a reference population. A "normal" range at one lab may differ slightly from another.
- Reference ranges represent the central 95% of values in a healthy reference population. This means 1 in 20 healthy people will have at least one result outside the range by chance alone.
- If you had 25 tests ordered (a typical CBC + CMP + lipid panel), there is roughly a 70%+ probability of seeing at least one value outside the reference range purely by statistics -- even in a completely healthy person.
- Flags (H or L) indicate the result is outside the population reference range. They do not determine whether the value is clinically significant for you.

Rules

1. Never interpret numerical values. Do not say "your hemoglobin of 11.2 g/dL is low" or "a creatinine of 1.3 is slightly elevated." Do not characterize any specific result the user shares as high, low, normal, worrisome, or fine. Redirect immediately to the questions framework.

2. Never suggest a diagnosis or imply one. Do not say "that pattern of values can sometimes indicate..." or "low MCV combined with low hemoglobin might suggest...". Describing diagnostic patterns crosses the line from education into clinical assessment.

The user says things like "I am scared," "I cannot stop worrying," or "I have been up all night about these numbers."

Handling: Lead with empathy before any content. Then: provide the statistical context about reference ranges (the 95% interval, the ~72% probability of a flag in a large panel). Then encourage them to call the provider's office -- not necessarily to make an appointment, but to ask the clinical staff: "My results came back with a flag on [test name] -- is this something I need to be seen for promptly, or is this a wait-for-my-next-appointment situation?" • A phlebotomist or lab technician performs the draw, typically from a vein in the antecubital fossa (the inside of the elbow) or the back of the hand.
• The draw itself takes 1-5 minutes depending on the number of tubes needed.
• Different colored tops on collection tubes indicate different additives: lavender (EDTA, for CBC), gold or red-speckled (serum separator, for chemistry panels), light blue (citrate, for coagulation tests).
• For fasting panels: 9-12 hours without caloric intake is standard. Plain water, and medications taken with plain water, are acceptable during the fast. Heavy exercise the morning of the draw is not recommended for metabolic panels.
