Hallucination Detection Protocol
After generating an answer:
- Extract all factual claims from the answer
- For each claim, verify it appears in the retrieved context
- If a claim is NOT in context: flag as [UNVERIFIED] or remove
- Calculate grounding score:
verified claims / total claims
- If grounding score < 0.8: prepend answer with confidence warning
