Output Format
For building forms, always produce a Python script that generates the JSON. Never hand-write raw JSON. The builder function pattern (in references/schema-rules.md) guarantees every required property is present, which is the only reliable way to prevent import failures. The script should write the .fulcrumapp file to disk.
For data events and calculations, produce the JavaScript directly with inline comments.
Clarifying Questions for Underspecified Requests
When someone says "build me a survey" without enough detail, ask about:
- Research objectives -- What decisions will this data inform? What questions need answering?
- Subject and population -- What/who is being surveyed? (households, schools, infrastructure, health) Who are the respondents?
- Key data points -- What specific variables need capturing? Any validated instruments or standard indicators to use?
- Language -- What language(s)? Default for ADF: Haitian Creole (Wi/Non, not Yes/No)
- Repeatable sections -- Any items that repeat? (household members, inventory rows)
- Conditional logic -- Fields that should appear only under certain conditions?
- Existing questionnaire -- Any Word doc, PDF, or draft to convert?
- Data events -- Auto-fill, cascading visibility, validation, or API lookups needed?
- Administration mode -- Enumerator-administered (CAPI) or self-administered? This affects question wording and description detail.
Don't ask all nine if context already answers some. Use judgment.
Questionnaire Design Methodology
Good questionnaire design is not intuitive. Research spanning decades shows that question wording, variable type selection, response scale design, and question sequencing all directly affect data quality. Before writing any Fulcrum field, the underlying research design decisions need to be sound.

Variable Classification Drives Field Type Selection
Every variable should be classified before choosing a Fulcrum field type. Apply two diagnostic tests:
- The Subtraction Test: If subtracting one record's value from another yields a meaningful result, the variable is quantitative. (Age: 30 - 25 = 5 years, meaningful. Religion: Catholic - Protestant = ?, meaningless.)
- The Mid-Way Test: For quantitative variables, if a midpoint between two values is logically meaningful, it's continuous. (Duration 10-13 min = 11.5 min, meaningful = continuous. Children 5-8 = 6.5, meaningless = discrete.)

The field type determines what analyses are valid downstream. Collecting age as ranges ("18-25", "26-35") in a ChoiceField when you could collect exact age as a numeric TextField throws away analytical power permanently. You can always group continuous data into categories during analysis, but you can never recover precision from pre-categorized data.
Question Wording
- Use simple, everyday language (under 25 words per question when possible)
- Ask one thing at a time (no double-barreled questions)
- Avoid leading questions and vague qualifiers ("often", "usually", "recently")
- Replace vague time references with specific periods: "In the last 7 days..." not "Recently..."
- Handle sensitive topics with care: ranges for income, normalizing introductions, placement later in the form
- Distinguish enumerator instructions from question text: labels are read aloud, descriptions are instructions
Questionnaire Sequencing (The Funnel Approach)
- Start with easy, rapport-building questions related to the survey purpose
- Group questions by topic using Fulcrum Sections
- Move general-to-specific within each topic (the "funnel")
- Place demographics and sensitive questions last
- Use screening questions early to route past irrelevant sections
- End interview-style forms with an open-ended closing question
Check-All-That-Apply vs. Forced Choice
When measuring multiple binary attributes (e.g., "Which of these services do you use?"), research shows that separate YesNoField per item produces higher quality data than a multi-select ChoiceField. With checkboxes, respondents tend to select fewer items. With forced choice, the enumerator asks about each item explicitly. Use forced choice when the list has 3-8 items and each matters independently for analysis.
Scale Design
For attitude/satisfaction/severity measurement: 5-7 point scales are optimal. Include numeric values in choice values for easier analysis. See references/questionnaire-design.md Section 3 for full guidance on midpoints, scale direction, and Fulcrum implementation patterns.
Critical Rule: Complete Schema Properties
Every field object MUST include the FULL property set Fulcrum expects, even when values are null or false. Fulcrum's import endpoint fails silently with a 500 error when any property is missing. No helpful error message, just a blank failure.
Before building any .fulcrumapp file, read references/schema-rules.md for the exact property templates per field type. That file was reverse-engineered from verified working exports and includes complete builder functions.

The visible_conditions_behavior key is ALWAYS "clear" whether conditions are active or null. Label fields do NOT have default_previous_value or ai_prompt -- including them causes import failure.

ADF Survey Design Standards
Mandatory conventions for ADF Haiti surveys. Follow unless the user explicitly says otherwise.
1. Field Descriptions (Enumerator Guidance)
Every data-collection field MUST have a description. The description appears as help text beneath the question on the mobile device. Its purpose is to ensure every enumerator interprets the question identically, regardless of experience or training level. A good description removes ambiguity so that two different people reading it will arrive at the same understanding of what data to collect.
What to include in a description:
- What the question is asking for in plain, specific language
- How to handle edge cases (e.g., "If the person rents from a family member, choose 'Lwaye' not 'Fanmi'")
- Units or expected format when not obvious (e.g., "Ekri distans la an minit mache, pa an kilomèt")
- Who to ask or how to observe (e.g., "Mande chèf kay la dirèkteman" or "Obsève epi verifye vizuèlman")
- Boundary conditions (e.g., "Si moun nan gen egzakteman 18 an, chwazi 'Wi'")
What NOT to put in a description:
- Repeating the label verbatim
- Generic text like "Enter the value" or "Select an option"
- Internal database notes or developer comments
Language: Write descriptions in the same language as the form (usually Haitian Creole for ADF). Keep sentences short and direct.

For CalculatedFields: descriptions are optional (enumerators don't interact with them directly, but a brief note about what the calculation shows can be helpful).
2. Standard Choice List Endings: "Lòt" and "Pa Aplikab"
Every choice list gets two standard options appended at the end:
- "Lòt" (Other) -- value
"Lot". Paired with a conditional TextField (use make_lot_text builder) that appears when "Lot" is selected. The TextField label appends " - Lot" and data_name appends "_lot". Do NOT use Fulcrum's allow_other: true.
- "Pa Aplikab" (N/A) -- value
"pa_aplikab". For when the question genuinely doesn't apply to the respondent's situation.
Use include_lot=True in make_choice to automatically append both options. The builder handles this -- you just need to add the make_lot_text follow-up field for the "Lòt" text entry.
When to omit: Only skip these if the user explicitly says to, or if the choice list is truly exhaustive with no possible "other" (rare -- e.g., biological sex: Fi/Gason).
3. Numeric Fields for Age, Years, Counts (Never Pre-Categorize)
Never use ChoiceField with ranges ("18-25", "26-35") for quantitative variables. Use a numeric TextField with min/max. This is not just a style preference -- it's a data integrity rule. Pre-categorizing continuous or discrete data permanently destroys analytical power. You can always group exact values into ranges during analysis, but you can never recover the original precision from ranges. Apply the Subtraction Test: if subtracting values is meaningful (age, count, income), it's quantitative and belongs in a numeric TextField.
Exception: For highly sensitive numeric data where respondents are likely to refuse (income in some contexts), ranges in a ChoiceField may improve response rates. This is a deliberate tradeoff -- discuss with the user.
4. Clean Conditional Labels
When a field is conditionally visible, do NOT include "if yes" or "si wi" in the label. The condition handles the logic; the label should stand alone.
5. Haitian Creole Accent Marks (Labels vs. Data)
Haitian Creole uses accented characters (è, ò, à, etc.) that are important for correct pronunciation. Enumerators need to see proper Creole spelling so they read questions correctly to respondents. But accented characters cause problems in data exports, column names, API queries, and downstream analysis tools.
The rule is simple: anything an enumerator reads aloud gets proper Creole accents; anything that becomes saved data or a column name does not.

Always use accent-stripped lowercase for choice values. Pass choices as (label, value) tuples to make_choice so labels keep their accents while values stay ASCII-safe:

The N/A option ("Pa Aplikab") is always on by default. It gives the enumerator a way to indicate when a question doesn't apply to the respondent, rather than forcing a Wi/Non answer that would be misleading. This is especially important in household surveys where not every question applies to every person.

9. Section Commentary Field
Add a free-text field at the end of every section for the enumerator to capture additional information, respondent comments, or their own observations that don't fit elsewhere. This is a safety net -- respondents often volunteer important context that falls outside the structured questions, and enumerators may notice things worth recording.

For survey forms that are interview or discussion-style (as opposed to pure observation or inventory), add an open-ended question at the very end of the form giving the respondent a chance to share anything that wasn't covered. This respects the respondent's voice and often surfaces the most valuable qualitative insights.

11. Paradata Fields (Auto-Populated)
Every survey form should auto-capture process metadata ("paradata") that enables quality control without adding burden to the enumerator. Use data events to populate these on new-record:

12. Pilot Testing Checklist
Before deploying any survey, pilot test with 15-30 respondents from the target population. Check:
- Do respondents understand every question as intended?
- Are response options exhaustive? (High "Lot" selection rates = missing categories)
- Does skip logic work correctly in all paths?
- How long does the full interview take? (Target: under 45 minutes for household surveys)
- Does the app perform well on target devices?
After piloting, revise questions, add missing choice options, adjust skip logic, and pilot again if changes are significant.

When using SETHIDDEN, always handle both change AND load-record events. Otherwise fields will be in the wrong visibility state when editing existing records.
Which to Use?
- Simple show/hide on one field: Either works.
visible_conditions is simpler.
- Show/hide + toggle required: Data events (SETHIDDEN + SETREQUIRED).
- Dependent dropdowns or dynamic choice filtering: Data events (SETCHOICES/SETCHOICEFILTER).
- Complex multi-step cascading: Data events give more control.

Repeatable Sections
For 1-to-many child records (household members, inventory items, inspection rooms). They go at the top level of form.elements, sibling to Section, NOT inside a Section.
Limits: ~60 child records per parent, ~35 fields per row. Avoid nested repeatables -- they cause export issues.
Typical household roster: name, relationship to head, sex, age/birth year, marital status, education, occupation (with Lot pattern).
Pre-Import Validation Checklist
Run through this before delivering any .fulcrumapp file:
Form level: name set, status_field present, record_title_key points to a valid field key, title_field_keys is a non-empty array, elements is non-empty
StatusField: key is "@status", data_name is "status", choices has at least one entry, default_value matches one of the choice values
Every field: key and data_name both globally unique (including inside repeatables), data_name is accent-free ASCII lowercase (no è, ò, à -- use strip_creole_accents() to convert), visible_conditions_behavior present (always "clear"), visible_conditions_type and visible_conditions both present (even as null), required_conditions_type and required_conditions present, ai_prompt present on all types except Label, description contains meaningful enumerator guidance (not null or empty) on all data-collection fields
ChoiceField: multiple present (even when false), each choice has both label and value, choice values are accent-free ASCII lowercase (no è, ò, à in values)

App Design Best Practices
Field count: New teams <= 50 fields. Experienced: up to ~600. Over ~1,400: split into multiple apps.
Repeatable vs. Record Link: Repeatable for finite, tightly-coupled children (floors, household members). Record Link for unbounded or many-to-many, or records shared across apps.
Bilingual labels (Kreyol/English): When forms need to be readable by both Creole-speaking field teams and English-speaking partners/donors, use forward-slash syntax:

Kreyol goes first (it's the primary field language), English follows the slash. This applies to field labels, section headers, and status field labels. Data names and choice values remain accent-stripped ASCII regardless.
Survey language: Most ADF surveys use Haitian Creole. Read references/haitian-creole.md when writing Creole form content -- it covers grammar, question formation, accent marks, and survey-specific vocabulary. Key points: Creole is phonetic (write what you pronounce), verbs don't conjugate (use tense markers like te, ap, pral), articles go AFTER the noun (kay la = the house), and Èske marks yes/no questions. The reference also includes common vocabulary for demographics, health, education, water/sanitation, livelihoods, and housing -- use it to ensure natural, consistent Creole across all form labels and descriptions.

Data Events Gotchas

NUMBERVALUE() does not exist in Fulcrum. Use NUM() to convert to number, or parseInt()/parseFloat() for manual parsing.

Calculated fields do NOT support the change event. You cannot write ON('change', 'my_calculated_field', ...). If you need to react when a calculated value changes, either listen to the source fields that feed the calculation, use validate-record/validate-repeatable, or check the value on load-record/load-repeatable.

SETVALUE() does NOT trigger change events. If Field B's change handler depends on Field A, and you programmatically set Field A with SETVALUE('field_a', value), Field B's handler will NOT fire. Call the handler function explicitly after SETVALUE:

SETHIDDEN() uses the data_name, NOT the key. The $ prefix is only for referencing field values in expressions ($field_name), not in function parameters like SETHIDDEN('field_name', true).

Choice field values vs labels: Always use CHOICEVALUE($field) for choice fields in data events, not VALUE('field'). The stored value and display label can differ.

Default values don't trigger events. The new-record event does NOT trigger change events for fields with default values. Handle all initialization logic inside the new-record callback itself.

Repeatable events need the repeatable data_name as the second parameter: ON('load-repeatable', 'repeatable_data_name', callback).

Always restore conditional visibility on load-record and edit-repeatable. If you use SETHIDDEN() in a change handler, you MUST also set the same visibility state in load-record (and edit-repeatable if inside a repeatable). Otherwise, existing records will show fields in the wrong state when opened for editing.
App Design Gotchas

Data shares expose ALL records and fields. Use Saved Filters (formerly Shared Views) for controlled, filtered sharing with external parties.

Photos in data shares expire after 24 hours. Plan accordingly for any external integrations or reports that embed photo URLs.

_created_duration on parent records includes time spent in repeatable editors. Each repeatable item also tracks its own duration independently. Don't double-count.

The .fulcrumapp format is just JSON. It can be created programmatically and imported at web.fulcrumapp.com/apps/import.
ADF Haiti-Specific Gotchas

GPS accuracy matters in rural Haiti. Always include SETLOCATION(CURRENTLOCATION()) on new records. Mountain terrain and sparse cell coverage mean GPS locks can take time -- remind enumerators to wait for a fix before saving.

Rating scale convention (1-5): 1-Tre Move, 2-Move, 3-Mwayen, 4-Bon, 5-Tre Bon. Use this consistently across all ADF assessment instruments for cross-survey comparability.

Data Shares let you expose Fulcrum data via public URLs in multiple formats.

Import/Export
Importing a form: Build the .fulcrumapp JSON, run the pre-import checklist, go to https://web.fulcrumapp.com/apps/import, upload and click CREATE APP.
Importing data: CSV or Shapefile, max 10,000 records per batch. Media goes in a ZIP alongside CSV.
Exporting: Apps > Export for .fulcrumapp templates. Data: CSV, Shapefile, PDF. Do NOT include full history when exporting for migration (creates duplicates).
Troubleshooting
500 Error on import: Almost always a missing property. Check every field against references/schema-rules.md. Common culprits: missing default_previous_value on non-Label fields, Label accidentally having default_previous_value or ai_prompt, missing display: "inline" on Sections, missing multiple on ChoiceField, missing neutral/neutral_enabled on YesNoField, wrong key on StatusField. Isolation technique: comment out half the form, test which half fails, narrow down.
Data events not firing: Event names: hyphenated lowercase (load-record not loadRecord). Field refs: data_name with $ prefix, not key. CONFIRM/PROMPT/REQUEST are async and need callbacks. Check for JS syntax errors in script.
Calculation not computing: Check for circular dependencies. Ensure $ prefix and correct data_name. Test with sample values in browser console.
Skip logic not working: field_key in conditions = the field's key, not data_name. value must match stored choice value (case-sensitive). Confirm visible_conditions_type is "all" or "any" when conditions are active.
Import creates duplicates: Exported with full history. Re-export without history.
