HITRUST CSF

Initial Assessment

HITRUST is often demanded by health plan customers in BAAs — confirm the driver for the assessment before scoping. If the file is missing, ask: who is requiring HITRUST (customer contract, internal), which assessment type is in mind, what systems and data are in scope, what frameworks the organization already maintains, and the target certification date.
HITRUST CSF Structure
HITRUST CSF (currently v11.x; the version increments as authoritative sources change) is a control framework specifically designed for health-relevant organizations. Its structure:
- Control categories / domains organize requirements (e.g., Information Protection Program, Access Control, Audit Logging and Monitoring, Endpoint Protection, Mobile Device Security, Wireless Security, Configuration Management, Vulnerability Management, Network Protection, Transmission Protection, Password Management, Education/Training/Awareness, Third Party Assurance, Incident Management, Business Continuity, Risk Management, Physical & Environmental Security, Data Protection & Privacy)
- Control references within each domain (numbered references)
- Implementation levels (1-5) that scale specifications based on organizational risk factors (e.g., volume of records, geography, regulated status). Higher levels add stricter or more numerous requirements.
The exact count of control references, domains, and required statements varies by CSF version and the assessment type — always work from the current MyCSF generated illustrative or live assessment, not from memory.

This mapping is the practical value: evidence collected for HITRUST often satisfies multiple frameworks. Conversely, organizations with mature SOC 2 / ISO 27001 programs can map existing artifacts into MyCSF.
Scoping
Scoping is the most important step. Errors propagate through the entire engagement.
Inputs
- Business activities in scope (e.g., SaaS hosting PHI for payer customers)
- Systems & processes that store, process, or transmit in-scope data
- Data types (PHI, PII, financial, payment cardholder data)
- Geographies that affect risk factors and regulatory overlays
- Volume of records / transactions
- Hosting (on-prem, cloud — and which CSP)
- Third parties (sub-processors, BAs) — and their HITRUST/SOC 2 status
Outputs
- A defined scope statement describing systems, data, processes, locations
- A tailored control set generated by MyCSF using the scoping inputs
- A data flow diagram, system architecture, and process inventory consistent with the scope statement
The certification covers what is in scope and nothing else. Customers reading the report look at the scope statement first.
Inheritance and the CSP Inheritance Program
For controls implemented by a Cloud Service Provider (AWS, GCP, Azure, etc.), HITRUST's Inheritance Program lets in-scope organizations inherit the CSP's assessed controls rather than re-evidencing them. The CSP publishes inheritable controls in MyCSF; the assessing organization claims inheritance per control.
Inheritance is a major effort reducer for cloud-native environments. Confirm:
- The CSP is participating and has a current report
- The inherited controls match how the system is actually deployed
- Shared responsibility for partially inherited controls is documented (you still own the customer's portion)
PRISMA Scoring (r2)
For r2 validated assessments, each requirement statement is scored across five PRISMA maturity levels:

Each level is scored as fully compliant, mostly, partially, somewhat, or non-compliant. The aggregated score must meet HITRUST's minimum to certify the requirement statement, and the assessment as a whole must meet a minimum to certify.
In practice, Policy and Procedure are documentation; Implemented is what auditors test; Measured and Managed are where many organizations lose points without a metrics program.
The 5-Step Assessment Process (r2)
- Define scope in MyCSF; HITRUST generates the tailored control set
- Readiness assessment (optional but common) — perform a gap assessment against the tailored set
- Remediate gaps (technical, policy, process, metric)
- Validated assessment — performed by an external Authorized External Assessor; evidence collected and tested; submitted to HITRUST
- HITRUST review and certification — Quality Assurance review, issuance of certification report
For r2, an Interim Assessment is performed at the 1-year midpoint to confirm continued conformance. Recertification at the 2-year mark. e1 and i1 cycles are 1 year, with no interim.
Corrective Action Plans (CAPs)
When specific requirements fall short, HITRUST may permit certification with CAPs — documented plans to remediate. CAPs have target dates and are tracked through the assessment lifecycle. Excessive CAPs can prevent certification; the threshold differs by assessment type. Plan to enter the validated phase with zero or few CAPs.

Most of these are Measured / Managed failures — the control is in place, but no metric proves it.
How HITRUST Relates to SOC 2
HITRUST and SOC 2 are complementary, not redundant.
- SOC 2 is an attestation under AICPA standards against the Trust Services Criteria (security plus optionally availability, processing integrity, confidentiality, privacy)
- HITRUST CSF is a prescriptive control framework specifically designed for health-relevant regulated data
- Many controls map across both
- Some buyers ask for both — HITRUST satisfies the deep prescriptive expectation; SOC 2 Type II satisfies an AICPA-style independent attestation
Evidence collected for one can heavily reduce effort for the other. HITRUST publishes a SOC 2 + HITRUST CSF "SOC 2 mapping" engagement type that combines the two reports.
When HITRUST Is Required
- Payer contracts — many U.S. health plans require HITRUST certification (often r2 within a defined timeline) from their vendors and downstream business associates
- Health-tech procurement — large health systems frequently include HITRUST as a preferred or required certification
- HHS recognized practices — HITRUST publishes mappings to "recognized security practices" relevant under HITECH-amended HIPAA penalty considerations
Confirm the customer's exact requirement: assessment type (e1/i1/r2), scope, timeline, and reporting (full report vs. letter of certification).
Practical Engineering Workflow
- Confirm the driver and required assessment type
- Inventory in-scope systems, data, processes, and third parties
- Run a MyCSF readiness assessment — get the tailored control list early
- Map each requirement to an evidence owner and an expected artifact
- Identify inheritable controls from the CSP
- Bake controls into the platform (IaC, identity, logging, vulnerability management, change control)
- Establish metrics for each major control to score Measured / Managed
- Run internal pre-assessment 30-60 days before the External Assessor engagement
- Submit to HITRUST after Assessor review; respond to QA
- Schedule the interim (r2) and the recertification
Common Anti-Patterns
- Treating HITRUST as "HIPAA compliance" — it is a control framework; HIPAA compliance still requires the rule-level work in hipaa-compliance
- Starting evidence collection only when the External Assessor engages
- Failing to enable inheritance and re-evidencing CSP-provided controls
- Skipping Measured/Managed — high score on Policy/Procedure/Implemented but no metrics program
- Over-scoping — bringing the entire enterprise in when only one product is required
- Under-scoping — excluding systems that customers expect to be covered, weakening the certification's market value
Output Format
When advising on a HITRUST question:
- Confirm the driver and proposed assessment type (e1/i1/r2)
- Walk through scope and propose adjustments
- List inheritable controls from declared CSPs
- Identify the riskiest control domains for the organization's current state
- Recommend a readiness timeline and milestones
- Flag any items that require an External Assessor's input
Do not invent control reference numbers (e.g., specific HITRUST CSF reference IDs) unless certain — refer to MyCSF for the current text. HITRUST publishes the canonical content.
Task-Specific Questions
- What assessment type is the target — e1 (foundational/entry), i1 (1-year, threat-adaptive), or r2 (2-year, comprehensive certification) — and what is driving the choice (customer requirement, board mandate, contract)?
- What is the scope — which products / business units / data flows / environments are in (and explicitly out), and is that scope already documented?
