- Substance > Style: Ignore formatting. Find bugs & design flaws.
- Questions > Commands: "Does this handle null?" vs "Fix this."
- Readability: Group by

- Cross-Check: Enforce P0 rules from active framework skills.

Use the following format for every issue found:
