STEP 1 — Gather Information

If safe → proceed to obtain off‑medication serum cortisol or UFC; if not safe → do not obtain off‑medication sample and consider alternative assessment (e.g., clinical evaluation only).
STEP 3 — Classify or Stratify
Interpret the off‑medication serum cortisol or UFC using the assay’s normal range: normal → radiation therapy effective and no adrenal insufficiency; elevated → persistent hypercortisolism or inadequate radiation effect; low → new adrenal insufficiency. → Classify the result as normal, elevated, or low.
STEP 4 — Decide
If normal: continue current follow‑up and repeat testing at 6‑ to 12‑month intervals. If elevated: consider intensifying radiation therapy, adding or adjusting medical therapy, or evaluating for surgical revision. If low: initiate physiologic glucocorticoid replacement (hydrocortisone 15‑20 mg/day divided), provide stress‑dosing education, and monitor for recovery of the HPA axis.
