The guideline suggests adding eicosapentaenoic acid ethyl ester 4 g/day to reduce cardiovascular disease risk in adults on statins with persistent triglyceride levels 150–499 mg/dL who have either established atherosclerotic cardiovascular disease or type 2 diabetes plus ≥2 additional risk factors. Typical triggers are “Patient on statin with TG 150-499 mg/dL”, “Statin-treated patient with ASCVD or diabetes plus 2 risk factors”, and “Moderate hypertriglyceridemia despite statin therapy”.
