Risk Register Best Practices
- Update weekly (minimum) during active projects
- Review in sprint planning (do any risks affect the sprint?)
- Review in retrospectives (did any risks materialize? Were our mitigations effective?)
- Close risks that are no longer relevant (don't let the register grow stale)
- Each risk has ONE owner (not "the team")
- Track when risks materialize (measures quality of risk identification)
Risk Response Strategies
The Four Response Types
1. AVOID: Eliminate the risk entirely by changing the plan
Example: Risk of using untested technology

2. MITIGATE: Reduce probability or impact through proactive action
