Create or review an Architecture Decision Record capturing decisions already made that affect system structure or quality attributes and are hard to reverse, including context, alternatives, and consequences. Use after an RFC is accepted, for major technology selection, migrations, or superseding existing architecture decisions.

起草、审查、收敛或更新辉夜计划RFC，适用于跨仓库、公共API/Schema、核心技术栈、AI/Agent权限、长期状态、具身控制、安全伦理、治理或难回滚的重大变更。
必须从 RFC 模板创建文档。写正文前填写 decision_scope、created、updated、last_reviewed、summary、canonical、related、supersedes、superseded_by；日期使用 ISO 格式，并保留已接受决策的历史关联。同一决策的 RFC 与 ADR 必须复用稳定的小写 kebab-case decision_scope。
本地文档关系使用相对路径，并把 RFC 接入既有治理文档图。related 必须双向，每个 supersedes 必须与旧文档的 superseded_by 配对。
moonweave-skills docs-index --root <repo> --format json，比较既有 RFC/ADR 的 decision_scope、标题、摘要、领域、状态和关系；CLI 不可用时，从 frontmatter 建立同等清单。lint-docs，重建链接索引，并用 docs-index --check 验证。优先使用以下紧凑结构：
