Calculate Health Score

Compute a deterministic health score from QA artifacts and determine the verdict.

Purpose

Take issue counts, coverage percentage, and phase statuses, then compute a health score (0-10) and verdict (SHIP / SHIP WITH FIXES / DO NOT SHIP / INCONCLUSIVE).
