GraphQL PENDING review whose commit.oid matches the head means a reviewer is still
composing feedback; return to the bounded polling path even when reviewRequests is
empty. APPROVED or COMMENTED means the pass posted, but every finding in its body
still must be addressed. Use the latest same-SHA submitted review per reviewer to decide

Separately carry change requests across head changes. For each reviewer, inspect all
