Code-specific diagnosis hints

These codes are frequently misunderstood or have non-obvious sub-classes. Use these hints to refine the lane and recommended action.

• 20 ENDORCTL_RC_INTERNAL_ERROR. Multiple distinct failures surface as "an internal error occurred in endorctl. Please contact Endor Labs for support". Inspect the next log line to discriminate sub-classes including a scan process that stopped before writing a terminal state (covered by the stuck STATUS_RUNNING pattern), sandbox or toolchain initialization failure (often shares text with codes 38 and 39), source-provider installation/auth failures, and scanner-specific upstream service errors. Recommend a single retry on the latest endorctl; if the failure persists, prepare a redacted support escalation packet rather than guessing root cause from sparse logs.

Use the next log line to choose between waiting for the in-flight scan, fixing the repo state, escalating the backend init failure, or upgrading endorctl.
• 27 ENDORCTL_RC_HOST_CHECK_FAILURE. The next line names the missing toolchain (for example "Cannot access the "pnpm" command"). Recommend installing that exact toolchain on the scanning host or switching to a CI image that includes it.

• 32 ENDORCTL_RC_SIGNATURE_VERIFICATION_FAILURE. Use the endorctl artifact verify workflow and look for ArtifactSignature evidence in Endor. For self-signed certificate chains, add the certificate to the scanning host's trust store; do not recommend skipping signature verification as a fix.
• 33 ENDORCTL_RC_LICENSE_ERROR. This is an Endor product entitlement signal, not a software-license finding. The most common cause is a feature (for example AI SAST) that is not enabled for the tenant. • 129 ENDORCTL_RC_POLICY_WARNING. The scan completed and a warning-only admission policy fired. Communicate that this is a warning, not a hard block, and review the policy if warnings appear unexpected.
• 135 ENDORCTL_RC_DEPENDENCY_SETUP_WARNING. Scan completed (often as STATUS_PARTIAL_SUCCESS) despite a non-fatal setup issue. If findings counts look lower than expected, fix the underlying setup warning rather than ignoring it.

When to recommend support escalation

Recommend a SUPPORT_ESCALATION_RECOMMENDED verdict when the exit code points at a back-end class of failure and tenant-visible evidence cannot narrow it further. Typical cases:

• 20 ENDORCTL_RC_INTERNAL_ERROR with sparse logs and no actionable details (after a single retry on the latest endorctl).
• 38 ENDORCTL_RC_TOOLCHAIN_ERROR or 39 ENDORCTL_RC_SANDBOX_ERROR recurring across multiple scans of unrelated projects in the same namespace.
• 25 ENDORCTL_RC_VULN_ERROR, 34 ENDORCTL_RC_HUGGING_FACE_ERROR, 36 ENDORCTL_RC_ARTIFACT_OPERATION_FAILURE, or 37 ENDORCTL_RC_SEGMENTATION_ERROR when the failure is not a user-side input issue.
• Stuck STATUS_RUNNING records with no terminal exit code (covered above).

Always assemble the support escalation packet with redacted scan UUIDs, workflow UUIDs, namespace, exit code numeric value and name, and the first few log entries. Never include secrets, tokens, or credential-bearing fields.

PR Scan Performance

For slow PR scans in a large repository or monorepo, check whether the scan is falling back to a full PR scan because no valid baseline is available or the changed content cannot be determined. Recommend incremental PR scans when the goal is faster PR feedback and the project has a stable baseline branch.

Useful public guidance to return when relevant:

• Enable incremental PR scans so only changed dependencies and changed code are scanned where supported.
• Provide or verify a baseline such as main when automatic baseline inference is unavailable.
• For CLI-driven scans, use the public shape: endorctl scan --pr --pr-baseline=<baseline_branch> --pr-incremental.
• For app-triggered scans, check the scan profile's automated PR scan, incremental PR scan, PR comment, language, path filter, and scanner settings.
• If no dependency-relevant files changed, an incremental PR scan may skip expensive work rather than behaving like a failed scan.

PR Scan Baseline And Shallow-Clone Diffs

Distinguish four common PR-scan failure modes before recommending an action:

• Shallow clone breaks diff computation. CI pipelines that perform a shallow checkout (for example a small clone depth) may make the merge-base between the PR head and the baseline branch unreachable. Endor cannot then compute the changed paths and either fails the diff step or falls back to a full PR scan. Recommend deepening the CI clone or explicitly fetching the baseline branch so the merge-base is in history.
• Stale baseline drift. PRs sometimes show findings that were not introduced by the PR itself when the baseline branch has not been rescanned recently. Recommend re-running the baseline scan and, where relevant, rebasing the PR on the latest baseline so the comparison is against current main.
• Missing prior baseline. PRs opened on a project that has no prior full scan of the default branch may produce no PR check, no comment, or no findings because there is nothing to compare against. Recommend a baseline scan on the default branch first, then re-trigger the PR.
• Silent policy block. An action policy can gate a PR before any user-visible scan output exists. From the developer's view, this looks like no scan ran. Inspect the matching policy's scope and disposition before assuming the scan itself failed.

Scan Lifecycle And Stuck States

Treat the following lifecycle states as distinct from ordinary failures:

• Stuck STATUS_RUNNING. A scan whose ScanResult.spec.status is STATUS_RUNNING long after the expected duration, with a stale update_time, sparse logs, and no terminal exit code, indicates that the scan process failed before writing its final status. Back-end housekeeping eventually transitions the record to STATUS_FAILURE, but reruns before that may collide with the stuck record. Recommend waiting for the terminal state, then escalating to Endor Support with the redacted scan result UUID, workflow result UUID, namespace, and the first few log entries if no useful cause is visible. Do not create a ScanLogRequest in V1.
• Parallel scan contention. An automated scan that begins while another automated scan for the same project is still running can fail with an initialization-stage error. Recommend waiting for the in-flight scan to finish, then retrying, and staggering CI triggers so duplicate concurrent scans for the same project are avoided.
• Sandbox/toolchain initialization errors. Errors that mention scanner-managed toolchain or sandbox setup, and that occur before dependency resolution begins, are usually not fixed by dependency-manager credentials or manifest edits. Retry once with the latest endorctl; if it persists, escalate to Endor Support with the redacted evidence packet.

Dependency Resolution

Classify ecosystem errors separately:

• NPM/Yarn/pnpm: private registry auth, .npmrc scope, lockfile drift, node version, invalid manifest, workspace, or lifecycle script issue.
• Maven/Gradle: private registry auth, Java version, Maven HTTP blocker, Android/Kotlin toolchain, Gradle config, local Artifactory, or compile issue.

• NuGet: private feed auth, SDK version, target framework, Windows or Visual Studio dependency, local source, or corrupt package issue.
• RubyGems/Bundler: private source auth, Ruby version, Bundler version, GitHub HTTP source, or gemspec issue.
• Packagist/Composer: auth token, custom repository, secure-http, invalid version, or lockfile drift.

Prioritize private registry and private source access when Endor evidence says dependencies were not downloaded. Then check toolchain/build-environment failures. If both are possible, report both hypotheses and the next evidence needed to distinguish them.

Additional ecosystem-aware patterns to recognize without inventing tenant evidence:

• NPM/Yarn/pnpm projects that check in a .npmrc that references an environment variable not present in the scanning environment surface as a warning about being unable to replace the environment value, followed by an unauthenticated request to the private registry. Recommend a Package Manager integration that supplies credentials directly, rather than relying on the env-var substitution in the project's .npmrc.
• Some private NPM-style registries reject the initial unauthenticated request rather than negotiating; configure the auth so credentials are presented on the first request, and verify the package manager integration for that registry is healthy in Endor before suspecting code or lockfile issues.
• A PackageManager integration that is configured for one ecosystem does not implicitly cover other ecosystems on the same private host. Confirm there is an explicit integration for each ecosystem in use, and that its package_manager_status is healthy.

Approximate Scans And Reachability Modes

Two distinctions matter when reasoning about findings:

• Approximate scans. When dependency resolution fails, Endor can fall back to an approximate analysis that estimates resolved versions from manifest data. Findings produced from an approximate scan can include false positives when the estimated version differs from what would actually be installed. PackageVersion.spec.precomputed_call_graph_state, the presence of resolution_errors, and non-zero ScanResult.spec.stats.dependency_analysis_num_approx are signals that the scan ran in approximate mode. Recommend fixing dependency resolution upstream rather than dismissing the findings.
• Traditional vs precomputed reachability. Traditional reachability analyzes the application's call graph plus its dependency call graphs to determine reachability of a vulnerable function. Precomputed reachability is used as a fallback when a full build is not possible; it conservatively assumes direct dependencies are reachable and only verifies the chain to vulnerable functions through cached open-source call graphs. Communicate which mode produced the evidence rather than implying function-level reachability for application code when only precomputed reachability is available.

Authentication And Namespace

If Endor auth fails, check for local configuration vs environment variable conflicts before recommending credential rotation. The user may have a local config file for single-namespace use or environment variables for CI and multi-namespace use. Do not print the config file. Safe checks include whether the file exists and whether the relevant env vars are set, without printing secret values.

For SSO, distinguish product identity provider configuration from the local auth method used by the current CLI or host. Report OIDC/SAML metadata, issuer, discovery URL, redirect, claim mapping, certificate, and tenant-selection evidence only when returned by read-only Endor queries or provided by the user.

Distinguish authentication (the SSO handshake itself) from authorization (the claim-to-namespace mapping). When a user successfully signs in but lands on a tenant-create or empty-tenant view, authentication succeeded and authorization failed: the claim values returned by the identity provider do not match what the namespace's auth policy expects. Ask the user for the claim names and example values they expect, compare them to the auth policy's required claims, and recommend aligning attribute mapping rather than rotating credentials. When namespaces are nested, also verify that authorization is propagated to the child namespace the user is trying to reach.

Toolchain And Host-Check

Treat host-check failures as their own lane before recommending Endor-side configuration changes:

• Required toolchain missing from PATH. Errors of the shape host-check-failure: Cannot access the "<tool>" command mean the scanning host does not have the named tool installed or on PATH. Recommend installing the missing tool on the scanning host, or running the scan on a CI image that includes it. This corresponds to host-check exit code 27.
• Toolchain version mismatch. Projects that pin a Node engine, Python version, Go SDK, or JVM newer than what the scanning host provides surface as package-manager errors such as Unsupported engine, ensurepip is not available, missing native headers, or transitive dependencies that require a newer SDK. Recommend pinning the appropriate toolchain in the scan profile, or scanning on a CI image whose toolchain meets the project's requirements.
• macOS or sandbox quarantine. On macOS, downloaded binaries may be blocked by quarantine attributes. Recommend the documented quarantine removal step before suspecting an Endor issue.

Endorctl Version Hygiene

Many surface-level scan errors are fixed in newer endorctl releases. Confirm the version with endorctl --version early in any diagnosis. When the running version is significantly behind the latest released endorctl, add a recommended action to upgrade to the latest stable release and rerun the failing operation before further root-cause investigation. Frame this as a low-friction step, not a guess, and prefer it over speculative scan-profile edits.

SCM Installation Drift And Sync Logs

When app-triggered scans or PR comments stop arriving from a source provider even though the installation appears configured, suspect drift between the Endor Installation record and the source-provider's installation:

• The app may have been removed on the source-provider side while the Endor record still exists, or removed on the Endor side while the source-provider app continues to deliver webhooks. Each can manifest as repeated webhook delivery errors and missing PR events.
• Recommend reviewing the integration's sync logs (available per Installation) for delivery errors, then re-installing or removing the orphan installation to restore parity.
• Confirm the source-provider app's permission set includes the events and scopes required for PR scans and PR comments: pull request webhook events read, and pull request comments read and write.

Container Scanning

For container scan issues, prefer the dedicated endorctl container scan workflow. Treat deprecated endorctl scan --container usage as a likely remediation path. Check image source, registry auth, registry type, scan plan counts, digest lookup errors, tarball mode, and whether a locally built image has a registry reference reachable from the scanning environment.

Be explicit about registry-level versus per-image scanning. Registry-level scanning of a whole container registry on a schedule is supported only for a subset of registry types. When a user expects "scan every image in our registry" against a registry type that is not yet supported for that mode, recommend scanning each built image during CI with endorctl container scan against the image reference, rather than waiting for registry-level support. Notifications And Exporters

When findings exist but downstream destinations (Jira, Slack, webhook, PagerDuty, custom HTTP) do not receive notifications, walk through these checks before assuming a delivery outage:

• Target presence. Confirm a NotificationTarget exists in the namespace that the finding belongs to. Targets configured only in a parent namespace may not reach a child namespace unless they are configured to propagate or duplicated in the child.
• Destination payload mismatch.
