Audit Process
Part A: Code Review
Review all changes against the hardened plan and guardrail files:
- Scope Compliance — All changes within the Scope Contract?
- Forbidden Actions — Off-limits files/folders touched?
- Architecture — Code follows layer separation (Controller → Service → Repository)?
- Error Handling — Proper error types, no empty catch blocks?
- Naming — Follows project naming conventions?
- Patterns — Follows existing patterns from
.github/instructions/?
- Testing — New features covered by tests?
- Security — Input validation? No secrets in code?
- Project Principles — If
docs/plans/PROJECT-PRINCIPLES.md exists: Core Principles respected? Forbidden Patterns absent? Technology commitments followed?
For each finding, assign severity:
- 🔴 Critical — Must fix before merge (security, data loss, scope violation)
- 🟡 Warning — Should fix (pattern drift, missing test, naming)
- 🔵 Info — Nice to fix (style, minor improvement)

First, run git diff --name-only to get the definitive list of all changed files. Then compare against the Scope Contract:
- Scope Creep — Work not listed in the Scope Contract?

- Non-Goal Violations — Work contradicting Out of Scope items?
- Forbidden Actions — Off-limits files/folders touched?
- Architectural Drift — Patterns conflicting with instruction files?

If any 🔴 Critical finding or drift is detected:

- Do NOT approve the changes
- Document findings in the plan's

- The Fix Issues handoff button will appear to switch to the Executor agent for targeted fixes
- After fixes, re-run this Reviewer Gate
Targeted Re-Review (after LOCKOUT fix)
When re-reviewing after a LOCKOUT fix, focus on:
- The re-executed slices and their changed files (primary audit)
- Integration points between fixed slices and adjacent slices (regression check)
- The specific 🔴 Critical finding(s) that triggered the original LOCKOUT (confirm resolved)
Full review of unchanged slices may be skipped, unless the fix introduced cross-cutting
changes (shared interfaces, database schema). If in doubt, do a full review.
Pass Protocol
If no critical findings and no drift:
- Verdict = PASS
- The Ship It handoff button will appear to switch to the Shipper agent
- The Shipper handles commit, roadmap update, postmortem, and push
- For Small/Medium phases (≤5 slices): shipping can continue in this same session — Session 4 is optional
- For Large phases (6+ slices): a separate Session 4 is recommended to avoid context exhaustion
OpenBrain Integration (if configured)

- Before auditing:

Termination Guard — LOCKOUT Loop Prevention
⚠️ Critical: The Reviewer Gate → Executor → Reviewer Gate loop is the highest recursion risk in the pipeline.

Escalation message after 2 failed cycles:
"Two LOCKOUT cycles completed without resolving all 🔴 Critical findings. Human intervention required. Review the ## Amendments section in the plan for details."
If chat.subagents.allowInvocationsFromSubagents is not set, fall back to the "Ship It →" or "Fix Issues →" handoff buttons — they carry context automatically.

- Only run read-only commands:
git diff, git log, git status, git show, build commands, test commands. Do NOT run destructive commands (rm, git reset, git push)
