Hypoglycemia Severity Classification
STEP 1 — Gather Information
Obtain point-of-care blood glucose (or CGM) value and assess for neuroglycopenic symptoms (e.g., confusion, drowsiness), neurogenic symptoms (e.g., diaphoresis, tremor), and whether the patient requires assistance to manage the event. Record glucose value and symptom/assistance status.
STEP 2 — Rule In / Rule Out
Is there altered mental and/or physical status requiring assistance? If yes, classify as Level 3 (severe hypoglycemia) and proceed to Decide; if no, proceed to evaluate glucose threshold.
STEP 3 — Classify or Stratify
If glucose <54 mg/dL (3.0 mmol/L), classify as Level 2; if glucose is between 54 and 69 mg/dL (3.0–3.9 mmol/L), classify as Level 1.
STEP 4 — Decide
For Level 1: administer 15–20 g fast-acting carbohydrate, recheck glucose in 15 min. For Level 2: administer 20–30 g carbohydrate; if unable to ingest safely consider glucagon if oral intake not possible. For Level 3: administer glucagon immediately, call emergency services, and monitor until recovery.
Clinical Guardrails / Mimics / Pitfalls
Do not delay treatment for suspected Level 3 while awaiting glucose confirmation; avoid overtreating with excessive carbohydrates causing rebound hyperglycemia; recognize that hypoglycemia unawareness may blunt neurogenic symptoms, relying more on neuroglycopenic changes; do not use glucose alone to exclude Level 3 if assistance is required.
Concrete Clinical Example
A patient with type 1 diabetes reports shakiness and sweating; point-of-care glucose is 48 mg/dL, and they are able to self-treat. No assistance needed. Classified as Level 2; given 20 g glucose tablets, glucose rechecked to 78 mg/dL after 15 min.
