STEP 3 — Classify or Stratify
Obtain a lipid panel (non-fasting acceptable for initial screening); measure total cholesterol, HDL-C, and triglycerides; calculate LDL-C using the Friedewald equation if triglycerides <400 mg/dL, or consider alternative methods if triglycerides are elevated; classify triglyceride levels as normal (<150 mg/dL) or elevated (≥150 mg/dL) and LDL-C as per risk categories.
STEP 4 — Decide
If triglyceride levels are elevated (≥150 mg/dL) or genetic dyslipidemia is suspected, repeat a fasting lipid panel; use the triglyceride and LDL-C values to inform cardiovascular risk assessment and guide further management per endocrine disorder–specific guidelines.
