Vitamin D supplementation for adults over 75
STEP 1 — Gather Information
Collect patient age and assess for conditions that substantially alter vitamin D physiology (e.g., chronic kidney disease, granulomatous diseases, malabsorption) or established indications for vitamin D treatment/testing.

Classify the patient as a candidate for daily lower-dose vitamin D supplementation (preferred regimen) over nondaily higher-dose dosing.

Recommend empiric vitamin D supplementation (e.g., 800–1000 IU daily) without serum 25(OH)D testing.
