Monitor dental hygiene and oral health during antiresorptive therapy
STEP 1 — Gather Information
Collect medication details (type, dose, start date), baseline oral health status (recent dental exam, symptoms such as pain, swelling, loose teeth, exposed bone), cancer type, renal function, vitamin D and glucocorticoid use, and patient‑reported oral discomfort.
STEP 2 — Rule In / Rule Out
Determine if the patient is currently receiving or about to start an antiresorptive agent (IV bisphosphonate or denosumab). If no, follow routine dental care; if yes, proceed to next step.
STEP 3 — Classify or Stratify
Assess for signs/symptoms suggestive of osteonecrosis of the jaw (exposed bone, persistent pain, swelling, infection, non‑healing extraction site). Classify as: (a) asymptomatic, or (b) symptomatic/suspicious.
STEP 4 — Decide
For asymptomatic patients, schedule a routine visual dental exam every 6 months and reinforce oral hygiene instructions. For symptomatic/suspicious patients, refer promptly to a dentist or oral surgeon for evaluation, defer invasive dental procedures until assessed, and notify the oncology team.
