Interpret LDL cholesterol for dyslipidemia screening
STEP 1 — Gather Information
Collect patient age, BMI percentile (using CDC norms), fasting lipid panel (LDL, HDL, triglycerides, total cholesterol), fasting status, and family history of premature cardiovascular disease or dyslipidemia.
STEP 2 — Rule In / Rule Out
Is the patient aged 2–19 years with BMI ≥85th percentile? If yes, proceed to lipid screening; if no, routine dyslipidemia screening is not indicated per guideline.
STEP 3 — Classify or Stratify
Classify LDL cholesterol: <110 mg/dL acceptable, 110–129 mg/dL borderline high, ≥130 mg/dL high.
STEP 4 — Decide
If acceptable, repeat lipid panel in 2 years if risk persists; if borderline high, reinforce therapeutic lifestyle changes and recheck in 1 year; if high, intensify lifestyle intervention, reassess in 3–6 months, and consider referral to lipid specialist or pharmacotherapy if LDL remains ≥130 mg/dL after lifestyle trial.
