When this lens applies

• You are choosing a model and a headline benchmark or "beat the frontier" claim is part of the case.
• You are reading a vendor launch, leaderboard, or comparison chart and treating the numbers as proof.
• Your project's expectations rest on a marketing page or model card, with no success rate measured on your own examples.
• You train or evaluate agents inside a simulated environment (a model that "is the terminal", a world model, a synthetic env).
• You are about to upgrade, swap, or auto-update an underlying model in production.
• You depend on the model's built-in knowledge for current facts, or on a big context window to act as memory.
• You picked a model without reading its card's intended-use and limitations sections.

Failure modes

Wrong-axis hype ("beat the frontier" — at WHAT?)

• How it happens: A headline says model X beat Claude/GPT and readers assume "better at the task I care about." The benchmark actually measured a different axis (e.g. how well it predicts terminal output — impersonation realism — not task-solving). The claim is true but sideways, and the benchmark is often the vendor's own.
• Warning signs: "Beat the frontier" with no statement of at what; benchmark authored by the same lab; viral framing outrunning methodology; nobody in the replies asks "beat them at what axis?"
• Mitigations: Name the exact axis the benchmark measures and confirm it matches your use case before believing it. Check who built the benchmark — independent vs. vendor. Separate "is the work real?" (often yes) from "does the headline hold?" (often no). Never act on a metric whose definition you haven't read.

Benchmark theater (self-reported, unauditable, apples-to-oranges)

• How it happens: The only rigorously-run score is the vendor's; competitor baselines are each provider's self-reported numbers under different harnesses. Claimed peer models may be inaccessible (can't be re-run), and proprietary methodology means no outsider can reproduce the result. Evidence gets backfilled with testimonials ("500 testers loved it").
• Warning signs: "Per provider reported" footnotes; comparison set includes unreleased/unavailable models; methodology marked proprietary; no reproducible harness; vibes standing in for measurement.
• Mitigations: Verify every baseline ran under the same harness and conditions — reject "everyone timed their own lap" tables. Confirm claimed peer models were actually accessible and actually run. Require auditability: if an outsider can't reproduce it, treat the number as a press release, not a measurement. User enthusiasm is a signal, never a benchmark.

Benchmark-as-success-rate fallacy (your task ≠ the test)

• How it happens: You read "95% on MMLU/coding eval", copy it into expectations as "right 95% of the time for me", and ship. The benchmark measures a standardized test the model is tuned for, not your three real tasks — which you never measured. Months later: "the model got dumber."
• Warning signs: Expectations justified by vendor numbers; nobody can cite a success rate measured on your own examples; perceived decline that is really a never-measured baseline.
• Mitigations: Measure success rate on YOUR task with YOUR examples — build an eval of ~20 real cases from your actual workload before shipping. Treat vendor benchmarks as a hint, never a promise.

Silent post-ship drift (regression found by customers, not by you)

• How it happens: A model upgrade, swap, or vendor-side auto-update changes behavior. Without a regression set you can't tell "it got worse" from anecdote, so you learn it from customers six months later.
• Warning signs: No saved eval to re-run on version changes; "it used to handle this" complaints; quality debates settled by opinion because there's no measurement.
• Mitigations: On every model upgrade/swap, re-run a regression set on real tasks to catch silent degradation before users do. Pin model versions where possible and gate upgrades behind the eval. Make "it got worse" detectable, not anecdotal.

Sim-to-real gap (trained inside a model of the world)

• How it happens: A model that simulates an environment is a high-quality impression, not the real thing — "a model of Bash is not Bash." Agents trained inside it inherit the simulator's mistakes and hallucinations as ground truth — "flawless at the airport that doesn't exist" — then fail confidently on the real system.
• Warning signs: Strong in-sim performance with no real-world validation; simulator fidelity asserted, not measured; "simulation beats reality for training" with no independent evidence.
• Mitigations: Validate sim-trained agents against the real system before shipping. Measure where the simulator diverges from real Bash/OS/API. Treat simulated success as a hypothesis, not a result, and budget for the cases the simulator hallucinates.

Orchestration mistaken for a frontier model (and its hidden tax)

• How it happens: A vendor packages a multi-agent router — a learned policy delegating to a pool of specialist models, sometimes with recursive self-calls — behind one API and one name, then markets the aggregate score as a single trained model at frontier parity. The architecture is only visible in the cited papers. Every delegated call multiplies tokens, latency, and dollars hidden behind that one name.
• Warning signs: "One API, one name" but docs describe routing/delegation/coordination; frontier-parity claim with no model card describing training; unexpectedly high bills/latency for a "single model"; quality that only holds on bounded, well-shaped tasks.
• Mitigations: Distinguish a trained frontier model from an orchestration layer — read the cited papers before repeating any "matched the frontier" claim. Count the real per-request cost and latency once you include every delegated agent and self-call, and benchmark it against calling the frontier directly on your workload. Ask whether quality depends on models you can't access or afford in production.

Cutoff / context / off-label blindness

• How it happens: Three independent properties collapse into "how smart is it." Context window = how much it can read at once (the desk), not memory of past sessions and not intelligence. Training cutoff = where its knowledge stops; past it, it confabulates rather than refuses. And the card documents intended use and limitations — which almost nobody reads — so off-label underperformance shows up as a production "surprise" that was pre-documented.
• Warning signs: Expecting a big-context model to "remember" prior conversations; confidently-wrong answers about new libraries, current prices, or recent events; "wait, the model can't do that?" moments about limits that were written down.
• Mitigations: Read the model card for training cutoff (usually bold) and add retrieval for anything newer — "don't ask the brain, hand it the document." Don't treat context size as memory or intelligence; put what it must know into the context. Read the limitations section first and confirm intended use matches yours.

Pre-mortem questions

1. The benchmark says we "beat" the frontier — beat them at what exact axis, and is that the thing our project actually needs?
2. Who built the benchmark, were all baselines run under the same harness, and are the claimed peers actually accessible and actually run?
3. Can an independent party reproduce this result, or is the method proprietary-by-design and therefore marketing?
4. What is this model's success rate on our task, measured on our own examples?
5. When we upgrade or swap the model, how will we detect it got worse at our real tasks — before customers do?
6. If we train or evaluate inside a simulator, where and how do we validate against the real system before shipping?
7. Is this a trained model or an orchestration layer, and what is its real per-request cost/latency vs. calling the frontier directly?
8. Do we know the training cutoff and where the retrieval layer is — and are we mistaking context size for memory or intelligence?

Mitigations checklist

• For any "beat the frontier" claim, write down the exact axis measured and confirm it matches your use case.
• Check who authored the benchmark (independent vs. vendor) and reject self-reported, different-harness comparison tables.
• Confirm claimed peer models were accessible and actually run, and that an outsider could reproduce the result.
• Build a ~20-example eval from YOUR real workload and measure success rate before shipping.
• Save that eval as a regression set; re-run it on every model upgrade/swap; pin versions and gate upgrades behind it.
• Validate sim-trained agents against the real system, and measure where the simulator diverges, before trusting them.
• Determine whether you're buying a trained model or an orchestration layer; count real per-request cost/latency vs. direct.
• Read the model card: training cutoff, intended use, limitations — first, not after the surprise.
• Add retrieval for any fact newer than the cutoff; never let context-window size stand in for memory or intelligence.
• Treat user testimonials and "vibes" as a signal to investigate, never as proof.
