Phoenix OTel Development

OpenTelemetry registration and provider lifecycle layer for Phoenix. Single source of truth for OTel configuration — other Phoenix packages delegate here rather than importing OTel packages directly.

Dual-module output (CJS + ESM). Tests use vitest.
