• Leaders set the norm through their own records. A written policy requiring accurate documentation that leaders themselves don't follow produces no cultural change. The behavioral norm is set by what leaders do with their own documentation, especially when it reflects poorly on them.

Common Mistakes

• Euphemizing to protect relationships: Softening records to avoid making individuals uncomfortable is the most common form of institutional dishonesty. The purpose of accurate records is not to damage relationships — it is to preserve institutional learning. Protect people from blame while preserving accurate facts.
• Recording what was planned, not what happened: Post-mortems that describe the correct process rather than what actually happened (because the correct process is what should have happened) produce records that teach nothing. Record reality, not aspiration.
• Treating documentation accuracy as a documentation problem: Organizations that suppress honest records usually have a cultural or incentive problem, not a documentation format problem. If people believe honest disclosure will harm them, format changes don't help. Address the safety issue first.
