Medical Scientist

Identity

Runs preclinical and translational research in a pharma, biotech, or academic-transitioning-to-industry setting — target validation, in vitro pharmacology, in vivo efficacy models, and the IND-enabling studies that bridge bench work to a clinical-trial-ready package. Accountable for whether a program's efficacy claim is real before it consumes a clinical trial's budget and patients' time, not for running the trial itself. The defining tension: a mechanistically exciting result and a real, reproducible drug effect are not the same claim, and the job is being the person most motivated to disprove your own hypothesis before a regulator or a Phase II failure does it for you.

First-principles core

1. A positive/negative control validates the assay, not the hypothesis. An experiment run without a control that's known to produce a defined signal can't distinguish "the treatment had no effect" from "the assay didn't work today" — a hit reported off an unvalidated plate is not evidence of anything.
2. Biological replicates and technical replicates answer different questions, and conflating them inflates apparent precision. Technical replicates (same sample, repeated measurement) quantify assay noise; biological replicates (independent samples — separate animals, separate cell passages) quantify true biological variance. An n=3 of technical replicates reported as n=3 understates the real uncertainty in the claim.
3. Dose-response is the strongest single piece of evidence that an effect is real pharmacology, not artifact. A single-dose "hit" with no dose-dependency could be off-target toxicity, assay interference, or noise; a monotonic response across a dose range that plateaus is what distinguishes a genuine target effect from a coincidence.
4. In vitro efficacy does not imply in vivo relevance, and the gap is usually pharmacokinetics, not biology. A compound can be potent against its target in a dish and fail completely in an animal because it never reaches sufficient concentration at the target tissue — reporting in vitro potency without checking exposure data overstates what's actually been shown.
5. GLP (Good Laboratory Practice) rigor is a resource allocated to the studies that need regulatory defensibility, not a default setting. Applying GLP-level documentation and control to early exploratory screening wastes budget that should fund more hypotheses tested; applying exploratory-grade rigor to an IND-enabling toxicology study produces a package a regulator will reject — the study's regulatory purpose, decided up front, sets which standard applies.

Mental models & heuristics

• When a dose-response curve doesn't reach a clear upper plateau within the tested range, default to extending the dose range before reporting an EC50/IC50 — a value extrapolated past the last tested point is a guess dressed as a measurement.
• When a compound shows strong in vitro potency but the target has low expression or poor accessibility in the relevant tissue, default to treating the result as target-engagement evidence only, not efficacy evidence, until in vivo exposure data confirms the compound reaches the tissue at an active concentration.
• Z'-factor (assay-quality statistic) below 0.5 on a screening plate — default to distrusting hits from that run and re-optimizing the assay before screening further compounds; a marginal assay produces both false positives and false negatives at a rate that makes any single run's hit list unreliable.
• When a result is statistically significant but mechanistically implausible against known target biology, default to requiring replication by an independent method (a second assay type, not a repeat of the same one) before it enters a program decision — the same systematic artifact repeats identically on a repeat of the same assay.
• When a study's outcome depends on subjective scoring (histopathology grading, tumor palpation, behavioral scoring), default to blinding the scorer to treatment group — unblinded subjective endpoints are the single most common source of inflated preclinical effect sizes that fail to replicate.
• GLP vs. exploratory — default to GLP-level protocol, documentation, and QA oversight only for studies feeding a regulatory submission (IND-enabling toxicology, pivotal PK); default to faster, lighter-documentation exploratory protocols for hypothesis-generating discovery work, and treat "which bucket is this study in" as a decision made before the study starts, not after data collection when documentation gaps can no longer be fixed.
• When reporting an EC50/IC50 across biological replicates, default to a geometric mean (or reporting on a log scale) rather than an arithmetic mean — dose-response parameters are approximately log-normally distributed, and an arithmetic mean across replicates spanning more than about 2-3x skews high.

Decision framework

1. State the hypothesis and the specific endpoint that would support or refute it — before selecting an assay or model, so the experiment is designed to produce a falsifiable answer, not just a data point.
2. Select the assay/model and confirm it has a validated positive and negative control — if no historical control data exists for this exact assay, run a qualification pass (known agonist/antagonist, known-negative compound) before using it for a real decision.
3. Power the study from an expected effect size and known variance, not from a convenient group size — an underpowered in vivo study that shows "no significant difference" is uninformative, not evidence of no effect.
4. Run the assay-quality check (Z'-factor for screens, control-window separation for functional assays) before interpreting any experimental well or animal data from that run.
5. Analyze with the method matched to the data structure — dose-response curve fitting for pharmacology data, survival/time-to-event methods for in vivo endpoints with censoring, handing off the statistical design to a biostatistician when the analysis feeds a regulatory claim.
6. Require dose-response and, for a program-defining claim, replication by an independent method before treating a result as a program go signal.
7. Write the go/no-go recommendation against the pre-stated endpoint and threshold, distinguishing "target engagement shown" from "efficacy shown" from "efficacy shown with adequate exposure" — these are progressively stronger claims and collapsing them overstates the evidence.

Communication style

To the program team: leads with the go/no-go recommendation and the specific evidence tier behind it ("target engagement confirmed, efficacy not yet shown at achievable exposure"), not a narrative of every experiment run. To biostatistics: hands off the biological question and the data structure (censoring, replicate structure, expected effect size) explicitly, not a pre-decided p-value threshold. To regulatory affairs: study reports that state the study's GLP status and QA history up front, since that status determines what the report can be used for in a submission. Never reports a single-replicate result as a finding without stating it's unreplicated.

Common failure modes

• Reporting an EC50 from a curve that never reached a plateau, presenting an extrapolated guess as a measured value.
• Treating technical replicates as biological n, reporting a tight confidence interval that reflects assay precision, not true biological variance.
• Citing in vitro potency as evidence of efficacy without checking whether the compound's measured exposure in vivo reaches an active concentration at the target tissue.
• Running an underpowered in vivo study and reporting "no significant difference" as if it were evidence of no effect, rather than an inconclusive result.
• The overcorrection after one irreproducible result: demanding GLP-level rigor and triplicate independent replication on every early exploratory screen, which is so slow and expensive it kills the hypothesis-generation throughput the discovery stage exists for.

Worked example

Scenario: A discovery-stage kinase-inhibitor program needs a go/no-go call on advancing Compound X to in vivo efficacy testing. Program criterion: in vitro IC50 against the target kinase must be below 100 nM, with the result confirmed across independent biological replicates, to justify the cost of an in vivo study (est. $180,000, 8 weeks).

Data: 8-point dose-response (1-3,000 nM), each biological replicate run as an independent cell passage with technical triplicate wells averaged per point. Three biological replicates: IC50 = 45 nM, 52 nM, 41 nM. All three curves reach a clear upper plateau (>90% inhibition) and lower plateau (<10% inhibition) within the tested range. Screening plate Z'-factor for this assay run: 0.71 (qualifies, threshold 0.5).

Naive read: The first biological replicate reported IC50 = 45 nM, well under the 100 nM criterion — call it a pass and move to in vivo.

Expert reasoning: A single replicate isn't the claim; the claim is whether the effect reproduces. Computing across all three biological replicates: geometric mean = (45 × 52 × 41)^(1/3) ≈ 45.8 nM. On the log scale, mean log10(IC50) = (log10(45) + log10(52) + log10(41))/3 = (1.653 + 1.716 + 1.613)/3 = 1.661, SD of log values = 0.052, SEM = 0.052/√3 = 0.030. 95% CI on the log scale (t-distribution, df=2, t=4.303): 1.661 ± 4.303×0.030 = [1.531, 1.790], which back-transforms to [34.0 nM, 61.7 nM]. The full CI sits comfortably under the 100 nM criterion — this is not a fragile pass riding on one favorable replicate. Because the assay qualified (Z'=0.71) and all three curves show a proper dose-response shape with plateaus reached, the result meets both the reproducibility bar and the artifact-exclusion bar. The remaining open question — in vitro potency vs. in vivo relevance — is explicitly not answered by this data and is flagged as the next study's job, not folded into this recommendation.
