Update the CHANGELOG

1. Determine the change type: Added / Changed / Fixed / Removed / Security.
2. Run scripts/next-version.sh (in this skill folder) to suggest the next semver.
3. Prepend the entry under the ## [Unreleased] section of CHANGELOG.md.
4. Keep each entry one line, imperative mood, referencing the PR/ticket.
