Format the changelog for a release

Bring the Unreleased section of CHANGELOG.md into release shape.

Steps

1. Read CHANGELOG.md and collect every entry under the Unreleased heading.
2. Group the entries by change type and order the groups as references/changelog-guide.md specifies.
3. Per references/changelog-guide.md, keep at most 12 entries per group; fold the overflow into a single "and N smaller changes" entry at the end of the group.
4. Replace the Unreleased heading with the release version and today's date, and add a fresh empty Unreleased heading above it.
5. Never rewrite entries under an already-released heading.

Output

Show the reformatted section and wait for the user to confirm before writing the file.
