Bemdec (Bempedoic Acid) — Prescribing Guide
Brand: Bemdec (India) | NEXLETOL (USA)
Class: ATP-citrate lyase (ACL) inhibitor — reduces hepatic cholesterol synthesis upstream of HMG-CoA reductase
Dose: 180 mg orally once daily, with or without food
Step 1 — Confirm the Indication
Bempedoic acid is indicated as add-on to maximally tolerated statin therapy in adults who need further LDL-C lowering with either:
- Heterozygous familial hypercholesterolaemia (HeFH), OR
- Established atherosclerotic cardiovascular disease (ASCVD)
⚠️ CV morbidity/mortality benefit has not been established. It lowers LDL-C, not proven outcomes (as of 2020 label).
Not approved in children (<18 years).
Step 2 — Check the Current Statin for Interaction
This is the most important safety check before prescribing.

Why: Bemdec raises simvastatin AUC ~2-fold and pravastatin AUC ~2-fold → increased myopathy risk. Atorvastatin and rosuvastatin elevations are minor (~1.7-fold) and do not require dose adjustment.
Step 3 — Screen Special Populations Before Starting

Step 4 — Start and Schedule Monitoring
- Start: 180 mg once daily with maximally tolerated statin
- Lipid check: Recheck LDL-C at 8–12 weeks after initiation
- Uric acid: Check at baseline and at 4 weeks (peak rise occurs in first 4 weeks)
- LFTs / CK: Check if symptoms develop — not required routinely unless symptomatic
Step 5 — Monitor and Manage Adverse Effects
🔴 Hyperuricemia (Gout Risk)
- Incidence: Uric acid rises in 26% of patients; clinically significant hyperuricemia in 3.5%
- Gout: 1.5% on Bemdec vs 0.4% placebo; rises to 11.2% in those with prior gout history
- Action:
- Asymptomatic rise: monitor periodically
- Symptomatic gout: start urate-lowering therapy (allopurinol/febuxostat); do not stop Bemdec unless severe
🔴 Tendon Rupture
- Incidence: 0.5% vs 0% placebo — rotator cuff, biceps, Achilles most common
- Risk factors: Age >60, corticosteroids, fluoroquinolones, renal failure, prior tendon disorder
- Red flags — STOP IMMEDIATELY: snap/pop sensation, acute pain, bruising over tendon, inability to move/weight-bear
- Yellow flags — consider stopping: new joint pain, swelling, or inflammation → switch to alternative lipid-lowering therapy
- Rule: Discontinue Bemdec at first sign of tendon rupture. Avoid in patients with prior tendon rupture history.

Platelet rise >100×10⁹/L: 10.1% — not clinically significant.
Step 6 — Patient Counselling Points
- Take once daily, with or without food
- Inform about gout risk — report severe joint pain, swelling especially in the big toe
- Inform about tendon risk — rest immediately and call if snap/pop/pain in shoulder, upper arm, or heel
- Alert on simvastatin/pravastatin muscle risk if co-prescribed
- Stop and call if pregnant
- LDL-C check at 8–12 weeks after starting
Clinical Guardrails
- Do not use as monotherapy — must be adjunct to maximally tolerated statin
- Do not combine with simvastatin >20 mg or pravastatin >40 mg
- Do not start in Child-Pugh C or ESRD — no safety data
- Do not ignore new musculoskeletal complaints — could be tendon warning
- Bempedoic acid does not interact with warfarin, metformin, or oral contraceptives
