Indication (adults)

Adjunct to a reduced-calorie diet and increased physical activity for chronic weight management in:

• BMI ≥ 30 kg/m² (obesity), OR
• BMI ≥ 27 kg/m² (overweight) with ≥ 1 weight-related comorbidity — hypertension, type 2 diabetes, or dyslipidemia.

Maintenance dose: 2.4 mg once weekly (SC).

Contraindications

• Hypersensitivity to semaglutide or excipients
• Pregnancy
• Personal or family history of medullary thyroid carcinoma (MTC) or multiple endocrine neoplasia type 2 (MEN 2)
• History of pancreatitis

Dose Escalation (once-weekly SC, every 4 weeks)

Adverse Effects

Red Flags — When to Stop Noveltreat

• Persistent severe GI intolerance
• Symptoms suggesting pancreatitis (abdominal pain with nausea/vomiting)
• Vision changes (possible retinopathy)
• Thyroid nodule or dysphonia — evaluate for MTC
• Serum calcitonin > 50 ng/L — evaluate further for MTC; evaluate thyroid nodules on exam or neck imaging

Overdose

• Signs: severe nausea, vomiting, hypoglycemia.
