Perform full physical and gynecological examination to exclude organic etiologies
STEP 1 — Gather Information
Collect history of pregnancy, galactorrhea, headaches, vision changes, thirst/urination, thyroid symptoms, androgen excess (hirsutism, acne, clitoromegaly), weight changes, exercise intensity, stressors, menstrual pattern, fractures, substance use, and family endocrine/reproductive disorders. Perform physical exam: weight, height, BMI, thyroid palpation, breast exam for galactorrhea, external genitalia for hymenal integrity, signs of androgen excess (hirsutism, acne, clitoromegaly, acanthosis nigricans, skin tags), and in selected cases (primary amenorrhea or suspected outflow tract anomaly) perform bimanual to assess uterine size, position, and adnexal masses.
STEP 2 — Rule In / Rule Out
Is pregnancy confirmed by positive hCG or clinical signs? If yes, rule out FHA (amenorrhea due to pregnancy) and initiate prenatal care. If no, proceed to physical exam evaluation.
STEP 3 — Classify or Stratify
If any abnormal physical exam findings suggest organic etiology (e.g., thyromegaly, galactorrhea, pelvic mass, hirsutism with acne/clitoromegaly, imperforate hymen, neurologic signs), classify as organic etiology suspected. If no such findings are present, classify as no organic etiology detected.
STEP 4 — Decide
If organic etiology suspected, order targeted investigations (TSH, prolactin, pelvic ultrasound, endocrine workup) and postpone FHA diagnosis. If no organic etiology detected, proceed with FHA evaluation (baseline labs, bone density, etc.) per guideline.
Clinical Guardrails / Mimics / Pitfalls
Do not rely on physical exam alone to exclude all organic causes; some require imaging or labs. Do not miss subtle signs like anosmia (Kallmann) or mild thyromegaly. Avoid mistaking physiologic variants (e.g., mild acne) for pathology. Perform bimanual only when indicated (primary amenorrhea, suspected outflow tract) to avoid discomfort. Always exclude pregnancy first before labeling amenorrhea as functional.
Concrete Clinical Example
A 19‑year‑old runner presents with 5 months amenorrhea, BMI 17.2, intense training. History denies pregnancy, galactorrhea, headaches, thyroid symptoms. Physical exam: no thyromegaly, no galactorrhea, normal external genitalia, no hirsutism or acne; bimanual (selected due to primary amenorrhea concern) shows normal uterus, no adnexal masses. No organic signs identified; proceed to obtain pregnancy test and baseline labs (TSH, prolactin, LH/FSH, estradiol) to evaluate for FHA.
